Hello,
Attached DS-DE 125 Form and Poll Watcher Guidelines.
* Please use the Duval DS-DE 125 as attached, provides columns for the first and last name.
* Please only list people who have been asked and agreed to be a poll watcher.
* Party attorneys must be listed on the poll watcher form to enter a polling place or early voting site.
Per Florida Statutes 101.131 each political party and each candidate may have one poll watcher in each polling room or early voting area at any one time during the election.
To designate your poll watcher(s), you must use the attached form prescribed by the Division of Election, revised for Duval County. Please do not use the pdf version. For the 2024 General
Election early voting, the deadline to submit your form designating your poll watchers is 10/07/2024 before noon. You may email your completed form to me at cfackler@coj.net <mailto:cfackler@coj.net

Once we receive your list of names, we will verify that each poll watcher designated is a registered voter in Duval County. If we have any questions, you will be contacted.
Our office will prepare badges that must be worn by your poll watchers while they are watching in a designated voting area. The badges can be picked up by the candidate/party/committee
or you can choose to ask each poll watcher to pick up his/her badge at our office downtown. If using the second option, the badge will be printed while the poll watcher is in the office.
