Dear Plan Review Applicant:
The Florida Department of Agriculture and Consumer Services (FDACS)-Division of Food Safety has
reviewed the plans for the above referenced Food Establishment. The plans appear to be in conformance
with Chapter 5K-4, Florida Administrative Code. The plans have been approved with the following
provisos:
• Construction finishes in areas of moisture (such as food/beverage preparation and 3-
comparment sink areas, etc.) must have smooth, easily cleanable, and non-absorbent floors,
walls, cove base and ceilings.
• The food protection equipment (such as sneeze guards/shields, display cases, etc.) must
effectively protect the displayed food from sources of contamination. The inspector will
verify the effectiveness of the food protection equipment during the initial inspection.
• Provide documentation of the municipal water/wastewater services from the provider
before the initial inspection (e.g., copy of the water/sewer utility bill, Interagency Evaluation
Form, etc.). The documentation must be uploaded when completing the online food permit
application. Failure to provide such documentation may result in delays with the food
permit and initial inspection process.
• Designate an area for the storage of maintenance and cleaning chemicals/equipment. This
must be a designated area away from food and food contact surfaces.
• Designate an area for employees to store their personal articles. This must be a designated
area away from food and food contact surfaces.
• A direct connection between a drain from a 3-compartment sink and the sewage system
may not exist unless approved by the local plumbing authority.
• Operator or designee is to check with the local fire authority for fire safety related
requirements such as number of exits, fire extinguishers, sprinkler systems, etc.
• Operator or designee is to check with the local building department for requirements related
to plumbing, electrical, mechanical, etc.
Approval of these plans are contingent upon a satisfactory onsite initial inspection.
Approval of these plans do not indicate compliance with any other codes, laws or regulations that may be
required including federal, state, or local.
Any changes to these plans should be approved by the Florida Department of Agriculture and Consumer
Services (FDACS)-Division of Food Safety. If you have any questions or require additional information,

When the project is complete and you have the approval from the local officials, please submit the food
permit application for the initial inspection twenty-one (21) days prior to opening at
https://foodpermit.fdacs.gov/. After receiving the request, the inspector will contact you within a few
business days to schedule the initial onsite inspection. If possible, please attach a copy of all the approved
plan review documents when submitting the food permit application.
