Tree Assessment
Water Oak Tree
The assessment below was performed in accordance with the Tree Risk Assessment BMP 2 nd
Ed. 2017, and as trimming and pruning will not reduce the risk of failure to persons and property
to below moderate, the rear 28” DBH water oak tree should be considered for removal, due to the
unacceptable degree of lean and the root plate lifting opposite direction of the lean, imminent
uproot potential is present. The surrounding fall zone should be barricaded to prevent possible
harm to persons and property.
Please, feel free to contact me at (904) 434-6427.

To Whom it May Concern:
Upon inspection of the property, I found a Hickory tree that should be removed. The tree
poses an unnecessary risk to person or property. The tree is suffering from root and collar rot. The
trunk of the tree has a large hole, and visible rot can be seen on the inside of the tree. The tree is
leaning over the neighbor’s property and is extremely hazardous to both properties. The tree should
be removed in order to reduce the risk of damage to the listed property and neighbor’s.
If you have any questions or concerns, please feel free to contact me.
Best regards,

Per your request we have visited the property at the above address on 2/20/2025 and
our findings are as follows:
An approximately 25”–28” DBH laurel oak (Quercus laurifolia) is located in the front
yard, within a cluster of trees to the left of the driveway. The trunk of this tree is
completely hollow, exhibiting extensive decay and has a significant decay pocket on
the trunk. The tree leans toward the road and nearby houses.
This tree is in decline and is a hazard. Targets within its fall zone include:
• Pedestrians
• Vehicular traffic
• Children playing in the roadway after 3 PM
• Multiple residential properties
Using the Tree Risk Assessment Qualification (TRAQ) matrix, based on a two-year
timeframe, this tree has been assigned a HIGH overall risk rating.
The detailed matrix findings are as follows:
• Likelihood of Failure: Probable
• Likelihood of Impacting Target: High
• Likelihood of Failure and Impact: Likely
• Consequences of Failure: HIGH
There are no mitigation options that will reduce the overall risk rating below a
moderate rating.
Please feel free to call me on my cell phone at 904-607-4113 with any questions.

Tree Assessment
Water Oak Tree
Mr. Manabot,
The below assessment was performed in accordance with the Tree Risk Assessment BMP 2nd Ed.
2017 and as trimming and pruning will not reduce the unacceptable risk of failure below moderate, the tree should be considered for removal.
The necessary removal of one 16” + 12” DBH water oak tree with a codominant trunk, located in
the rear right corner of the home, due to week attachment points of leads and limbs at multiple
bifurcation points of the codominant trunks. The above tree issues cannot be corrected by
trimming or pruning. Due to the species of the tree and the likelihood of failure that could cause
harm to persons and property during a normal weather events within two to five years. The
above tree be considered for removal.

To Whom it May Concern:
Upon inspection of the property, I found a Southern Magnolia tree that should be removed.
The tree poses an unnecessary threat to person or property. In recent weeks the tree has dropped
several large dead branches. Two of them landing in the road. At first look the tree did not appear
to be damaged in any way. An aerial lift was brought in to inspect the tree. After a closer look, I
found the top of the tree was severely rotted and the tree was infested with carpenter ants. The
entire top crown of the tree has been rotted out and the damage is spreading down the main trunk
of the tree. The tree is extremely hazardous to the property and surrounding area. The tree should
be removed ASAP.
If you have any questions or concerns, please feel free to contact me.

To Whom it May Concern:
Upon inspection of the property, I found a Cedar tree that should be removed. The tree
poses an unnecessary risk to person or property. The tree was severely damaged in Hurricane
Helene. A large section of the tree broke off and landed on the neighbor’s shed. The tree also has
several other broken branches stuck in the tree that has to be removed. The remaining canopy of
the tree has started to turn brown and die. The tree is leaning on a fence and grows directly over
the neighbor’s property. The tree is a hazard to the property and should be removed.
If you have any questions or concerns, please feel free to contact me.
