Mayor – Seat 1 (At‐Large )
Commissioner – Seat 2 (District 1308)

The first two forms needing to be filed with me are the DSDE 9 and DSDE 84 forms, which are attached
to this email.
Please Note: As mentioned, if the paperwork is filed with me sometime in March, a treasurer’s report
will be due no later than April 10, 2020 at 5 pm (see below). Forms used for the Treasurer’s Report are
available through the link provided above.
