Install (3) Helical Piers to the existing support columns, pier depth 14' to stabilize Entry Patio
using standard brackets.
Repair concrete up against building & re-paint concrete to match existing.
Proposed price for the above project:$9,321.43
This proposal is valid for 60 days. Payment Terms: 30%Due Upon Signing. 70%Due Upon Completion.
Pricing includes materials, labor,transportation, associated fees and taxes. All debris to be removed

Dear Mr. Sellers:
I have reviewed the settlement issue at Building 8 — Units 79, 80, 81, and 82 at 1085 Atlantic
Boulevard, Atlantic Beach, FL 32233 (Building 8)and have the following comments:
1. The structure is a two-story common brick apartment building, built in 1972.
2. There is a concrete block fire wall separating Building 8 into three individual areas with
four apartments in each area.
3. Units 79, 80, 81, and 82 are in the far north area of Building 8.
4. The floor is a concrete slab on grade.
5. The foundation is a concrete footing.
6. The front porch of the two upper units, Units 80 and 82, is starting to separate from the
main building due to settlement.
7. There is noticeable separation between the concrete block column and concrete beam of
the porch, due to settlement. (see photos)
8. The cause of the settlement is unknown, but most likely due to soil erosion and
decomposing organics in the supporting soil.
9. Soil borings were not taken.
10. It is likely that the settlement will continue to occur unless the foundation is stabilized.

11. To stabilize the porch, we recommend installing 1 three inch helical pier along each of the
three concrete block columns supporting the porch, for a total of 3 three inch helical piers.
12. Each helical pier shall be up to 14 feet long using standard brackets and spaced as shown
on the attached sketch.
13. The design is based on the Florida Building (Residential) Code, 7`h Edition (2020).

Proposal for Stabilizing 2" Floor Entry Patio
Lloyd's Construction & Consulting, LLC proposes to provide turnkey construction services to complete
the below mentioned project.
Proposed Scopes of Work:
Provide engineering& permitting for project.
Install (3) Helical Piers to the existing support columns, pier depth 14' to stabilize Entry Patio
using standard brackets.
Repair concrete up against building & re-paint concrete to match existing.
Proposed price for the above project:$9,321.43
This proposal is valid for 60 days. Payment Terms: 30%Due Upon Signing. 70%Due Upon Completion.
Pricing includes materials, labor,transportation, associated fees and taxes. All debris to be removed
daily. (1) Year Labor warranty provided. Any additional work resulting in billable amounts requested
shall be priced to owner as a change order and must be authorized in writing by project coordinator
prior to Lloyd's Construction&Consulting, LLC commencing. By signing this proposal the owner agrees

Dear Mr. Sellers:
I have reviewed the settlement issue at Building 8 — Units 79, 80, 81, and 82 at 1085 Atlantic
Boulevard, Atlantic Beach, FL 32233 (Building 8)and have the following comments:
1. The structure is a two-story common brick apartment building, built in 1972.
2. There is a concrete block fire wall separating Building 8 into three individual areas with
four apartments in each area.
3. Units 79, 80, 81, and 82 are in the far north area of Building 8.
4. The floor is a concrete slab on grade.
5. The foundation is a concrete footing.
6. The front porch of the two upper units, Units 80 and 82, is starting to separate from the
main building due to settlement.
7. There is noticeable separation between the concrete block column and concrete beam of
the porch, due to settlement. (see photos)
8. The cause of the settlement is unknown, but most likely due to soil erosion and
decomposing organics in the supporting soil.
9. Soil borings were not taken.
10. It is likely that the settlement will continue to occur unless the foundation is stabilized.

11. To stabilize the porch, we recommend installing 1 three inch helical pier along each of the
three concrete block columns supporting the porch, for a total of 3 three inch helical piers.
12. Each helical pier shall be up to 14 feet long using standard brackets and spaced as shown
on the attached sketch.
13. The design is based on the Florida Building (Residential) Code, 7`h Edition (2020).
