Boulevard, through the services of the Philippine Nurses Association of Northeast Florida, Inc.
has been forwarded to me for zoning review. This activity is permitted within the Commercial
General Zoning District, in which this property is located. In order to approve your City of
Atlantic Beach business license, however, the City needs to be provided with some additional
information. You have indicated that these services are provided under the Florida Department
of Health, Volunteer Health Care Provider Program. Please provide the City with all licensing
and certification related to that program. Also, please provide documentation confirming that
any heath care services will be provided only by those persons properly certified and licensed to
provide such services. Once this information is provided, that City will be able to issue your
