We will provide labor, material and permit to run 4" Schedule 40 sewer line and tap into 6" city lateral.
Backhoe service will be used to facilitate the job. Ditches will be backfilled to grade. We will contact
Central Locating to mark utilities. Please allow 3 to 5 days for locating once contract is signed.
We will permit through the Duval County Health Department to abandon the septic tank; it will be
pumped out and then inspected by Health Department officials and filled with clean fill after the
inspection.
We will install owner or city-provided meter on existing well and provide and install one (1) hosebibb
before the meter.
All permits included in bid price of job.
DWV to be Schedule 40 PVC. Water piping to be CPVC. Water taps, sewer taps and any other city
charges (impact fees) must be paid before work commences. Patchwork and landscaping are not
responsibility of plumber. Driveway/parking area must be vacant to begin job. All work areas to be
clear of storage stuff.
A signed copy of this contract must be in our possession before the mentioned work commences.
Freight charges on special order items to be paid for by owner/builder.
Christy First Coast Plumbing, Inc. will warranty our craftsmanship (installation only of products and
materials) for one year after completion of job. Products and material provided by Christy First Coast
Plumbing, Inc. will carry the manufacturer's own warranty as applicable on defective products. The
term "defective" shall not be construed as embracing damage that arises from misuse, negligence,
Acts of God, normal wear and tear or failure to follow operating or cleaning instructions.
Warranty work will be performed during regular working hours between 8:00 a.m. and 5:00 p.m.,
Monday through Friday, except on holidays.
We propose to furnish material and labor in accordance with the above prices and specifications for
the sum of Two Thousand Four Hundred Eighty Two Dollars ($2,482.00). This price is for work
performed during normal business hours; no overtime included.
DRAWS: to be paid upon job completion.
Draws to be paid upon job completion. If not paid, a service charge of 1.5% of the unpaid balance
per month (18% annum) plus all cost of collection, including attorney's fees incurred in the event of
legal action will be assessed. No plumbing will be performed unless or until prior billings for this job
have been paid.
The above prices, specifications and conditions are satisfactory and are hereby accepted. You have

Please be advised, Atlantic Beach Code Enforcement has found your property referenced above
to be in violation of the City of Atlantic Beach, Code of Ordinances, Chapter 22, Section 22-74
Installation of toilet facilities required: connection of facilities to public sewer to wit:

(c) The owners of all properties described in (a) above where public sewer is not
currently available, but becomes available in the future, shall connect their
toilet facilities to public sewer as follows:
(1) Within ninety (90) days after date of official notice from the city to do so.
Official notice shall be complete upon mailing said notice to the property
owner at the property address and at his or her last known address, if any.
Official notice was sent to you on July 24,2008 and March 29, 2011. This letter requests the
noted violations be corrected by payment of system development charges, obtaining the
services of a licensed plumbing contractor to make connection to the public sewer with
inspections and abandon and fill septic tank as required within ninety (90) days of this
notice.

A re-inspection will be done on or before August 15, 2011. To avoid having this case be
referred to the Code Enforcement Board, all listed violations on this notice must be in
compliance on or before the date established by Atlantic Beach Code Enforcement. The Board
may impose fines up to two hundred fifty ($250.00) per day for continuing violations. Upon
completing the corrective action(s) required, it is your responsibility to contact Atlantic Beach
Code Enforcement and arrange for an inspection to verify compliance.
It is our goal to enforce the codes and ordinances of the City of Atlantic Beach and protect the
health, safety and welfare of the City, and accordingly, your cooperation in this matter is greatly
