The contract for street sweeping expires July 31, 2009. USA
Services of Florida, Inc. (Mr. Carmine Latanza, Secretary)
has agreed to extend the contract for one year; at the current
prices. Sweeping is a requirement of our NPDES/MS4
pem~it, and our maintenance contract with FDOT requires
monthly sweeping of Atlantic Blvd & Mayport Road. This
contract was awarded in 2007 and may be renewed up to
two times. This is the second renewal.
BUDGET: Stormwater account number 470-0000-538-3400 contains
$28,000.00 budgeted for Street Sweeping.
RECOMMENDATION: Extend the contract for one year at the current cost, which is
$26,160.00.
ATTACHMENTS: Letter signed by USA Services of Florida, Inc. agreeing to
extend the contract.

Dear Mr Latanza:
As you are aware, the contract with the City of Atlantic Beach for Street sweeping expires on
July 31, 2009. We would like to extend the contract for another year at the current prices, if this
is acceptable to your company. If acceptable, please sign below and return to me before June 30,
2009, along with a current Insurance Certificate naming the City of Atlantic Beach as certificate
holder. Once I receive these I can prepare a staff report for the City Commission to accept the
extension.
We look forward to recommending that we continue our contract with your company.
