This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami-Dade County Product Control Division and accepted
by the Board of Rules and Appeals(BORA)to be used in Miami Dade County and other areas where allowed by
the Authority Having Jurisdiction(AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami-Dade County Product Control

Hi Don:
First, here is NuAir's Structural Report for the 500 SGD. The professional
engineer's signature is located on Page 4 of 4. Also, I have included some
tables and information on SGD from the AAMA/NWWDA 101/I.S.2-97
specification. The "R" represents a residential rating which means that the
SGD minimum panel width was 2'10" and 6'6" frame height. Any product
with the commercial rating for SGD begins at a minimum panel width of
3'10" and minimum frame height of 6'10". Therefore, the commercial
rating is a much more demanding rating than the residential.
Also, we are a certification agency only, that means we review test reports,
drawings, and bills of materials from independent test laboratories, validate
them, inspect the product at the manufacturing location to see that they
comply with the product that was tested at the laboratory and permit them to
label the product if they meet all requirements. Here is our listing for The
Department of Housing (HUD) as an approved inspection/certification
agency. We are also approved by NFRC, IGMA and Dade County. If you

members shall not move from their original position, in
relation to the glazing materials,by more than the original
glazing bite. The load for vertical rails shall be 70 Ibf
(320N), and 50 Ibf(230N) for all other rails.
2.2.19.5.3 Life Cycle Testing (AW designated doors
only)
When tested in accordance with AAMA 910,there shall be
no damage to fasteners, hardware parts, or any other
damage which would cause the window to be inoperable,
and air infiltration and water resistance tests shall not
exceed the Gateway Performance Requirements specified
in Table 2.1.

HUD BUILDING PRODUCT STANDARDS AND CERTIFICATION PROGRAM
FOR FENESTRATION PRODUCTS (WINDOWS AND DOORS)
GENERAL - ADMINISTRATION:
This certification program shall be administered by organizations
that qualify under the Administrator Qualifications and
Procedures of the HUD Building Product Standards and
Certification Program, and shall comply with 24 CFR 200.935,
except as modified by the provisions of this Use of Materials
Bulletin 111 (UM 111) . Copies of Administrator Qualifications
and Procedures for the HUD Building Products Standards and
Certification Program may be obtained from:

The procedures for carrying out the certification program for .
fenestration products (windows and doors) shall be in accordance
with 24 CFR Section 200.935 and this UM which supplements 200.935
by providing that:
1. The Administrator shall issue to the manufacturer,
labels containing the Administrator's mark, the
manufacturer's statement of conformance to UM 111,
the manufacturer's name, and plant location.
2. The certification label shall be affixed to each
fenestration product.
3. At least every four years, the Administrator shall
visit the manufacturer's facility to select a
commercial sample for testing in a laboratory
approved by the Administrator.
4. The Administrator also shall review the quality
assurance procedures twice a year to assure that
they are being followed by the manufacturer.
STANDARDS:
All fenestration products (windows and doors) shall be designed,
manufactured, and tested in compliance with the following
American Architectural Manufacturers Association (AAMA) standard.
AAMA/NWWDA 101/I.S. 2-97 "Voluntary Specification for
Aluminum, Vinyl (PVC) , and Wood Windows and GlAss -Doors"

Dear Mr. Humphries:
As you have been advised by our staff, the City Commission of the City
of Atlantic Beach granted your request for water service at 1931, 1933
and 1935 Beach Avenue. The approval was granted subject to the
provision that all costs be absorbed by you and that all lines be
installed as approved and subsequently inspected by the City of Atlantic
Beach.
As a matter of policy, we only install 3/4" water meters. .Additionally,
we would prefer that 1933 and 1935 Beach Avenue receive their own 3/4"
meters. If this is not possible, it will be necessary to bill the
property owner for the water consumed by the tenants at those locations.
You may contact our Building Department regarding the appropriate
connection charges and water meter charges.

The City Manager reported that the bid opening was held on Jan,4,198
at 2:00 p.m. Two bids were received and opened - Dickerson of Fla, ,
Inc. in the amount of $35,195. and W7U Paving, in the amount of
$35,525. The tabulation of bids is attached hereto and made a part
hereof. The low bid submitted by Dickerson does comply with the
terms and conditions of the specifications. The City Manager recom
mended award of bid to the low bidder. Mr. gloss was asked if he
had pursued the possibility of going through the City of Jacksonvill
for resurfacing bids, and he said yes, but the City of Jacksonville
was not interested.

Consulting Engineers - Saltair Drainage Study.
The 1983-84 Capital Improvements Program has allocated $4,000 for an
engineering study of the Saltair Subdivision Storm drainage system.
Mr. 11oss reported that England, Thirsts & Pliller have submitted a
proposal to conduct an engineering evaulation and drainage study
in the areas within Saltair and along the Selva marina lagoon to the
weir structure near Mayport Rd. The City Manager recor.Ynended that
England, ThiTns & Miller be authorized to perform the engineering
evaulation. Mayor Howell expressed the opinion that he believed a
study had already been done several years ago, and it was found that

A. Ordinance No. 5-83-11 - Second and Final Reading-Public Hearing
AN ORDINANCE TO PRESCRIBE AND AUTHORIZE A UNIFORM TRAVEL POLICY AND
PROCEDURE FOR THE CITY OF ATLANTIC BEACH OFFICERS, EMPLOYEES AND
AUTHORIZED INDIVIDUALS; AND PROVIDING AN EE'FECTIVE DATE.
Said Ordinance was presented in full, in writing for second and
final reading by Mayor Howell. Said Ordinance was posted in line
with Charter requirements. Mayor Howell then opened the floor for
a Public Hearing. As no one spoke for or against, Payor Howell
declared the Public Hearing closed.

During discussion before the vote, Commissioner Morris asked if Van Ness x x
Section 6, page 3 - requiring receipts for meals was intended to Howell x
be left in. The City Manager answered yes. The question was called
and the iiotion carried unani-iriously.

AN ORDINANCE PRESCRIBING RULES AND REGULATIONS FOR THE PURCHASE OR
CONTRACT FOR SUPPLIES, MATERIALS, EQUIPMIVr, SEM7ICES, AVID PUBLIC
IMPROVETETIS; REIDEALING SECTION 72 OF THE CHARTER FZ?TITLED "CONTRACT
FOR CITY D1PROVE,E: TTS", AND ARTICLE II, SECTION 8-4 THROUGH 8-8 OF
THE CODE OF ORDINANCES =ITLED "PURCHASING".
Said Ordinance was presented in full, in writing on first reading
by Mayor Howell.

During discbefore the vote, Playor Howell asked t-he City Van Ness x
Attorney if he had determined if Section 72 of the Charter could be Howell x
repealed by home rule. City Attorney Mullis replied that Section 72
of the Charter can be repealed by home rule. The question was called
and the motion carried unanimously. Public Hearing was set for
January 23, 1984.

1,1-iyor Haaell sadly announced that Judae Charles Anderson, our
municipal judge from 1958 until the courts were abolished, had
sassed away Sunday night.
Mr. I�bss announced that the Department of Transportation will be
doing a study on traffic signals at I'ayport Road and Levy Rd this
rmnth.
There being no further business, the Mayor declared the meeting
adjourned at 7:53p.m.
