I met with the City's Building Inspector, the Public Works Division Chief and the Distribution Division Chief at the site. We made a physical inspection of the lateral on-site, on 1st St and in the manhole. The actual connection was uncovered and inspected. Mr. Bennett has also discussed the problems with me in my office on Wednesday, Thursday and Friday ( 10/14, 15, & 16) . On Wednesday he provided a survey report and on Friday he provided a corrected report. He was informed to make no effort at correcting problems until I had studied the information gathered. I also informed him that I would only make recommendations after discussing the matter with you. There appear to have been several City Ordinance violations which should be considered. 1. Sec. 22-106 Conformance with Building and Plumbing Codes. The lateral does not have minimum slope when applying the invert elevations shown on the two surveys. (Min. 1/8th inch per foot ) Excavation, Placing the pipe, jointing, testing and back-filling were not observed . 2. Sec. 22-109 Connections--- The connection to existing sewers was un-professional; " Hammer-tapped " and partially grouted to a active lateral serving the opposite side of 1st. St. It is not acceptable. 3,-S66. 22-110 Inspections--- To the best of my knowledge, no one on the City staff was notified that the work was authorized or that it had commenced. Page -2 (Bennett) 4. Sec. 22-192 Construction of Extension Projects To the best of my knowledge, there were no plans prepared for that portion of the lateral in the public right-of- way nor was there any approval; verbal or written. 5. Sec. 22-195 Construction Standards The PVC pipe found in the public right-of-way is 4" Schedule 40 material (a medium to lower thickness) with less than 20" of cover from the top of the pipe. City specifications require SDR-35 sewer pipe if PVC and a minimum of 30" cover in un-paved areas. (See extended copy of survey - Not official) After many discussions with various sources, I recommend the following: A. Require the removal of all sewer lateral pipe in the public right-of-way. B. Require the city-supervised repair of the six (6") inch sewer lateral to lot 11 "H". Approximately 6' will require replacement. (See exerpt from detailed plan of existing sewer) C. Consider the possibility of revoking Mr. Bennett's Builder's license for the listed violations. Sewer service to the property could probably be provided in one of two ways. 1. Have Mr. Bennett apply for the authorized septic tank. Telephone inquiries indicate that he has not applied. 2. Have Mr. Bennett contact an engineer to confirm the possibility of extending the existing 8" sewer main to a point opposite the centerline of his property on 1st St. to terminate with a standard clean-out. At this point he could probably connect a 6" lateral to provide for minimum slope. Requiring the 8" pipe would allow for less slope than smaller pipe. Susan Tripp requesting renewal of the audit contract between Deliot e, Haskins and Sells and the City of Atlantic Beach for a fee of $12,000. )0 Miss Tripp requested Commission approval for renewal of their audit co - tract for the third year in the amount of $1.2,000.00 in accordance with their engagement letter,dated July 3, 1985, which was approved by the City Commission at the meeting on June 24, 1985. The firm sent a letter to Mayor Howell on August 6, 1987 summarizing the scope of their proposed services which were basically the same as the prior two years. Motion: Renew the audit contract for the third year with Deliotte, Cook x Haskins and Sells at the price stated. Edwards x x Gulliford x x During discussion, Commissioner Gulliford asked if the City Manager, Howell x Finance Director, or anyone else interested were satisfied with their services. Mr. Fellows responded he was very pleased with the firm. The latter asserted that one report asked for last year was not received, but he expressed the opinion that mis-communication was the reason. The City Manager asked for the Mayor's comments, but Mayor Howell declined. The question was called and the motion carried with a three to one vote. Mayor Howell voted nay. 5. Application from Reuben and Janie Bennett for exception to the sewer moratorium in Section H Mrs. Janie Bennett asked for Commission approval for an exception to the sewer moratorium in Section H due to a hardship because of their under standing that sewer was available. Mr. Fellows said there was a misund r- standing between Public Service Director Braid and Mr. Bennett relativ to their ability to hook up to the city sewer system. Mr. Bennett boug t the property thinking utilities were available; however, when he went o city hall,city officials advised him that without a great deal of expe se to raise the grade of the lot and house, he could not tie into the exist- ing manhole at the next block. Mr. Fellows told the Commission Mr.Benn tt had agreed,that when the sewer line became available, he would disconn ct the septic tank and hookup to the city line. Commissioner Gulliford ex pressed concern on the present elevation; Mayor Howell told him Mr.Ben ett was right on the verge of the east/west flow. Motion: Move to grant an exception to the sewer moratorium to Cook x Reuben and Janie Bennett based on information furnished Edwards x x by the City Manager relative to the Bennetts agreeing to Gulliford x x hook up to the city's sewer line when available. These additions would increase the potential of a greater area to be served in the vicinity of the sewerline and pump station but the small capacity of the pumps and forcemain would restrict this potential As an alternative , we recommend that you consider construction of the ultimate West 3rd Street pumps and forcemain in accordance with the Section H Master Plan and possibly spur growth to this area for a better potential to recoup City funds used for construction. It is also my understanding the low pressure system will have to connect directly into the existing manhole at the foot of Camelia Street at its intersection with West First Street and said line must be installed a sufficient depth below the surface of the street to eliminate any possibility of breakage due to the passing of traffic.
