INFORMATION. THE OWNER HAS PROVIDED (SUV) SUBSURFACE UTILITY VERIFICATION REPORTS
SHOWING LOCATION OF EXISTING UTILITIES. THE SUV LOCATIONS IDENTIFY POINTS AT WHICH
THE UTILITIES HAVE BEEN LOCATED. THE EXISTING UTILITIES WERE EXTENDED BETWEEN THESE
POINTS TO PROVIDE A GENERALIZED REPRESENTATION OF THE PIPE ROUTE. HOWEVER, NO
WARRANTEE IS MADE THAT THE REPRESENTATION IS ACCURATE OR THAT ALL UTILITIES HAVE
BEEN LOCATED AND CHARACTERIZED. IT SHALL BE THE CONTRACTORS RESPONSIBILITY TO
EXPLORE AND INVESTIGATE FAR ENOUGH IN ADVANCE OF PIPE INSTALLATION TO PHYSICALLY
LOCATE, VERIFY AND CHARACTERIZE ANY UTILITIES; ALLOWING THE ABILITY TO MAKE IN -FIELD
ADJUSTMENTS TO AVOID CONFLICTS. THE COSTS FOR CONFLICT RESOLUTIONS SHALL BE THE
RESPONSIBILITY OF OF THE CONTRACTOR. THE OWNER AND ENGINEER SHALL NOT BE HELD
RESPONSIBLE FOR INACCURACIES OR OMISSIONS IN THE LOCATIONS OR GRADE OF THE
EXISTING FACILITIES REPRESENTED. ANY DAMAGE RESULTING TO THE EXISTING UTILITIES, EITHER
ABOVE OR BELOW GRADE, FOR FAILURE TO PHYSICALLY LOCATE THE UTILITIES SHALL BE
REPAIRED BY THE CONTRACTOR AT HIS EXPENSE.
6. CONTACT ALL UNDERGROUND UTILITIES REPRESENTATIVES AT LEAST 48 HOURS PRIOR TO
CONSTRUCTION.
7. CONTRACTOR SHALL BE RESPONSIBLE TO MAINTAIN MINIMUM CLEARANCE BETWEEN ALL
UTILITIES PER PLANS AND SPECS. CONTRACTOR SHALL BE RESPONSIBLE TO FURNISH AND
INSTALL REQUIRED FITTINGS AND RESTRAINTS IN ORDER TO ROUTE PIPES AROUND OTHER
PIPES AND OBSTRUCTIONS
8. HORIZONTAL AND VERTICAL CONTROLS ARE SUBJECT TO ADJUSTMENTS IN THE
FIELD IF NECESSARY TO AVOID UTILITY CONFLICTS UPON APPROVAL FROM THE
ENGINEER AT NO ADDITIONAL COST TO THE OWNER.
9. WHERE THE VERTICAL DISTANCE BETWEEN WATER AND SEWER MAIN CROSSING IS
LESS THAN EIGHTEEN (18)INCHES, THE CONTRACTOR SHALL ENCASE THE SEWER
MAIN IN CONCRETE FOR A DISTANCE OF TEN (10) FEET ON BOTH SIDES WHICH
MAY BE DISTURBED BY CONSTRUCTION, AND TO ASSURE ACCURATE HORIZONTAL
OF THE CROSSING.
10. THE CONTRACTOR SHALL EMPLOY A LAND SURVEYOR, REGISTERED IN THE STATE
OF FLORIDA, TO REFERENCE AND RESTORE PROPERTY CORNERS AND LAND
MARKERS DAMAGED DURING CONSTRUCTION AND TO ESTABLISH ACCURATE
HORIZONTAL AND VERTICAL CONTROL DURING THE CONSTRUCTION OF THE WORK.
11. ALL PIPE SHALL BE LAID WITH A MINIMUM OF 36" COVER UNLESS OTHERWISE NOTED.
12. PIPE JOINT DEFLECTION, WHERE REQUIRED, SHALL NOT EXCEED 80% OF THE MAXIMUM
RECOMMENDED FOR THE SIZE AND TYPE OF JOINT BY THE DUCTILE IRON PIPE RESEARCH
ASSOCIATION FOR DUCTILE IRON PIPE OR 80% OF THE MANUFACTURER'S RECOMMENDED
MAXIMUM ALLOWABLE PIPE DEFLECTION FOR P.V.C. PIPE. IF THE PIPE MANUFACTURER/SUPPLIER
RECOMMENDS A ZERO(0%) DEFLECTION FOR THEIR PIPE, THE CONTRACTOR SHALL BE
RESPONSIBLE TO PROVIDE ALL FITTINGS AND RESTRAINTS REQUIRED TO OBTAIN THE OFFSET OR
DEFLECTION BEING IDENTIFIED AT NO ADDITIONAL COST TO THE OWNER.
13. ALL MECHANICAL JOINT PIPE, FITTINGS AND VALVES SHALL BE RESTRAINED WITH RETAINER
GLANDS (SIZE, TYPE AND NUMBER AS REQUIRED).
14. VALVE BOXES FOR BURIED VALVES SHALL HAVE CONCRETE RINGS WITH BRASS TAGS INDICATING
SIZE, TYPE (PV OR GV) & DESCRIPTION (RW, WM, ETC.) AND NUMBER OF TURNS TO CLOSE/OPEN.
15. THE CONTRACTOR SHALL RESTORE ALL CULVERT, HEADWALL AND STORM DRAIN INLETS
REMOVED OR DISTURBED BY THE CONSTRUCTION OPERATION. THE COST OF THE ITEM
SHALL BE INCLUDED IN THE PRICE BID FOR FURNISHING AND INSTALLING ANY NEW
ITEM CAUSING SUCH DAMAGE.
16. TEMPORARILY STORE ALL EXCESS EXCAVATED MATERIAL AT CITY OWNED PROPERTY
LOCATED ADJACENT TO THE MASTER LIFT STATION AT WWTP#1. ALL UNSUITABLE
MATERIAL SHALL BE DISPOSED OF IMMEDIATELY BY THE CONTRACTOR AND AT HIS
EXPENSE. FINAL DISPOSAL OF EXCESS DIRT SHALL BE THE RESPONSIBILITY OF THE
CONTRACTOR AND AT HIS EXPENSE.
17 CONTACT CITY OF ATLANTIC BEACH AT LEAST 48 HOURS PRIOR TO ANY
ACTIVITY THAT WILL AFFECT WATER & WASTEWATER TREATMENT OPERATIONS,
INCLUDING TAPPING OF ANY MAINS.
18. THE CONTRACTOR SHALL PROVIDE A FINAL SURVEY AND AS-BUILTS OF ALL CHANGES MADE
TO THE ORIGINAL DRAWINGS. THE SURVEY SHALL BE IN STATE PLANE COORDINATES
NAD83 FL EAST ZONE FEET & VERTICAL NAVD 88 FEET . THE SURVEY AND AS-BUILTS SHALL
BE SIGNED AND SEALED BY A LICENSED FLORIDA SURVEYOR OR PROFESSIONAL ENGINEER AND
PROVIDED IN AN ELECTRONIC FORMAT (AUTOCAD V.14 OR GREATER) TO THE CITY OF ATLANTIC
BEACH.
19. CONTRACTOR TO COMPLY WITH CITY OF ATLANTIC BEACH SITE MANAGEMENT
ORDINANCE. COORDINATE WITH CITY OF ATLANTIC BEACH FOR ORDINANCE.

GENERAL NOTES FOR WATER AND SEWER INSTALLATION
1. THE CONTRACTOR SHALL BE RESPONSIBLE FOR DETERMINING THE EXACT LOCATION OF ALL UTILITIES AND FOR
)AMAGE TO EXISTING UTILITIES CAUSED BY HIS OPERATION. CALL UTILITY CONTACTS AND SUNSHINE STATE
DNE-CALL SERVICE(800-432-4770) 48 HOURS IN ADVANCE OF WORK.
2. THE CONTRACTOR SHALL VISIT THE JOB SITE PRIOR TO BIDDING TO FAMILIARIZE HIMSELF WITH JOB SITE
CONDITIONS.
3. CONTRACTOR SHALL ARRANGE FOR AND PAY FOR ALL TESTING AS REQUIRED IN THE CITY OF ATLANTIC BEACH
AND/OR JEA SPECIFICATIONS PERTAINING TO THE JOB.
4. ALL WATER AND SEWER WORK SHALL BE AS PER CITY OF ATLANTIC BEACH STANDARDS AND SPECIFICATIONS
JNLESS SHOWN OTHERWISE. THE CITY OF ATLANTIC BEACH REFERENCES JEA SPECIFICATIONS WITH
EXCEPTIONS AS SHOWN ON THE PLANS, OR IN THE NOTES.
5. JTILITY CONTRACTOR SHALL NOT COMMENCE WORK WITHOUT PRIOR NOTICE TO THE CITY OF ATLANTIC BEACH
PUBLIC WORKS DEPARTMENT (247-5834). TWO DAYS NOTIFICATION IS REQUIRED BEFORE ANY LINES ARE
CONNECTED TO THE EXISTING UTILITY SYSTEM.
6. ALL WATER MAINS SHALL HAVE A MINIMUM OF 30" COVER FROM FINISHED GRADE IN UNPAVED AREAS AND 36"
MINIMUM COVER IN PAVED AREAS. WATER AND SEWER MAINS ARE DESIGNED TO FINISHED GRADE AND SHALL
BE PROTECTED FROM DAMAGE UNTIL ALL WORK IS COMPLETED.
7. HORIZONTAL AND VERTICAL SEPARATION SHALL BE AS SPECIFIED IN CODE 62-555.314 (F.A.C.) LOCATION OF
PUBLIC WATER SYSTEM MAINS:
(1) HORIZONTAL SEPARATION BETWEEN UNDERGROUND WATER MAINS AND SANITARY OR STORM
SEWERS, WASTEWATER OR STORMWATER FORCE MAINS, RECLAIMED WATER PIPELINES, AND ON-SITE
SEWAGE TREATMENT AND DISPOSAL SYSTEMS.
(A) NEW OR RELOCATED, UNDERGROUND WATER MAINS SHALL BE LAID TO PROVIDE A
HORIZONTAL DISTANCE OF AT LEAST THREE FEET BETWEEN THE OUTSIDE OF THE WATER
MAIN AND THE OUTSIDE OF ANY EXISTING OR PROPOSED STORM SEWER, STORMWATER
FORCE MAIN, OR PIPELINE CONVEYING RECLAIMED WATER REGULATED UNDER PART III OF

TEES AND OTHER FITTINGS AS SHOWN IN JEA CONSTRUCTION DETAILS W-31, W-37 AND S-38. MECHANICAL
RESTRAINTS TO BE MEGALUG TYPE.
9. UNSUITABLE MATERIALS UNDER WATER AND SEWER MAINS SHALL BE REMOVED AND REPLACED WITH SELECTED
BACKFILL, PROPERLY COMPACTED TO 95% OF MAXIMUM DENSITY.
10. THE CONTRACTOR SHALL COORDINATE THE WORK WITHIN THE CITY RIGHTS-OF-WAY WITH THE PROPER AGENCIES
FOR MAINTENANCE OF TRAFFIC AND METHOD OF CONSTRUCTION AND REPAIR.
11. ALL POTABLE WATER AND SEWAGE FORCE MAINS 4 INCHES AND LARGER SHALL BE C-900 DR 18, CLASS 150,
PVC OR DUCTILE IRON, CLASS 150, OR BETTER, UNLESS OTHERWISE NOTED ON PLAN SHEET.
12. ALL GRAVITY SEWER MAINS SHALL BE SDR 26 PVC CONFORMING TO ASTM D3040 OR DUCTILE IRON CLASS 50,
CONFORMING TO ANSI A21.50, AS REQUIRED, OR BETTER.
13. ALL GATE VALVES SHALL BE NON -RISING STEM TYPE AND SHALL BE SUITABLE FOR 250 PSI NON -SHOCK WORKING
PRESSURE. GATE VALVES SHALL BE MECHANICAL JOINT OR FLANGED, VALVES SHALL BE IRON BODY, BRONZE
FITTED. VALVES SHALL BE CLOW, M&H, MUELLER, OR APPROVED EQUAL. VALVE BOXES WITH SCREW
EXTENSIONS SHALL BE OF CAST IRON CONSTRUCTION, 3/8 INCH MINIMUM WALL THICKNESS AND SHALL BE NON -
TACKY TAR ENAMEL COATED. THE WORD "SEWER" SHALL BE CAST IN THE COVER FOR VALVES ON FORCE MAIN.
ALL GATE VALVES INSTALLED SHALL OPEN BY TURNING TO THE LEFT (COUNTER -CLOCKWISE) WHEN VIEWED
FROM THE TOP. VALVES SHALL HAVE RESILIENT SEATS.
14. HYDROSTATIC AND LEAKAGE TESTING, AND DISINFECTION OF THE WATER MAINS INSTALLED SHALL BE
PERFORMED IN ACCORDANCE WITH AWWA STANDARDS AND FAC CHAPTER 62-555 . A REPRESENTATIVE OF THE
CITY OF ATLANTIC BEACH MUST BE PRESENT DURING THE HYDROSTATIC AND LEAKAGE TEST. THE TEST IS TO
BE CERTIFIED BY A REPRESENTATIVE OF THE DESIGN ENGINEER.
15. FLUSHING AND DISINFECTING OF WATER MAINS TO BE COORDINATED WITH CITY OF ATLANTIC BEACH PUBLIC
UTILITIES DEPARTMENT.
16. BACTERIOLOGICAL TESTING OF WATER MAINS TO BE PERFORMED BY DOH CERTIFIED LABORATORY, PAID FOR
AND ARRANGED BY CONTRACTOR. COORDINATE ALL ACTIVITIES WITH CITY INSPECTOR.
17. NO CONNECTION TO EXISTING POTABLE WATER SYSTEM SHALL BE ALLOWED UNTIL ALL PROPOSED WATER
LINES HAVE BEEN PRESSURE TESTED, DISINFECTED AND CLEARED FOR SERVICE.
18. POST CONSTRUCTION TELEVISION INSPECTION AND 5% DEFLECTION TEST SHALL BE REQUIRED ON ALL GRAVITY
SEWER. A FULL REPORT AND TAPE OR CD AS TO CONDITION OF THE PIPE WITH PERTINENT DATA SUCH AS
DISTANCE BETWEEN MANHOLES, LOCATION OF SERVICES, ETC., SHALL BE SUBMITTED TO THE ENGINEER AND
CITY OF ATLANTIC BEACH. ANY DEFECTS ENCOUNTERED SHALL BE REPLACED OR REPAIRED PRIOR TO FINAL
ACCEPTANCE. ALL REPAIRED SECTIONS MUST BE REINSPECTED PRIOR TO ACCEPTANCE.
19. SHOP DRAWINGS ARE REQUIRED AND MUST BE APPROVED BY THE CITY BEFORE CONSTRUCTION.
20. ALL INSPECTIONS AND TESTING SHALL BE COORDINATED WITH THE CITY.
21. THE CONTRACTOR SHALL SECURE ALL NECESSARY PERMITS TO WORK IN EXISTING RIGHTS-OF-WAY.
22. ALL DISTURBED AREAS SHALL BE SODDED AND MULCHED, OR SEEDED AS DIRECTED, AND AT A MINIMUM
RESTORED TO PRECONSTRUCTION CONDITION.
23. EXISTING ASPHALT SHALL BE SAW -CUT IN STRAIGHT LINES FOR ALL TRENCHES. ANY DAMAGE TO CITY STREET
PAVEMENT, CURB, SIDEWALK, OR OTHER MATERIAL SHALL BE REPAIRED AS DIRECTED AND APPROVED BY THE
DIRECTOR OF PUBLIC WORKS. DENSITY TESTING MUST BE COORDDINATED WITH THE PUBLIC WORKS INSPECTOR
24. THE CONTRACTOR SHALL FURNISH AND INSTALL FITTINGS AS NECESSARY TO ROUTE PIPES AROUND OTHER
PIPES OR OBSTRUCTIONS AS REQUIRED.
25. ALL EXISTING UTILITIES HAVE BEEN SHOWN ON THE PLANS FROM THE BEST AVAILABLE INFORMATION. THE
OWNER SHALL NOT BE HELD RESPONSIBLE FOR INACCURACIES OR OMISSIONS IN THE LOCATIONS OR GRADE OF
FACILITIES OF THIS TYPE, THE CONTRACTOR SHALL BE HELD RESPONSIBLE FOR LOCATION OF ALL UTILITIES IN
