Enclosed is the State Revenue Sharing application for fiscal yeaz 1998-99. The City is required to file an application in order to be considered for any funds to be distributed under the Revenue Sharing Act. These revenue distributions for the City of Atlantic Beach include Sales Tax, Gas Tax, Cigarette Tax and the Motor Fuel Tax Rebate. These taxes represent $1,287,462 or 23.2% of the total General Fund budget and $417,969 in budgeted Gas Taxes for street related expenditures. The deadline for this application is Juue 30, 1998. Section 218.26(4), Florida Statutes, states the following: It shall be the duty of each agency and unit of local government required to submit certified information to the Department pursuant to the administration of this act to file timely information. Any unit of government failing to provide timely information required pursuant to the administration of this part shall, by such action, authorize the department to utili2e the best information available or, if no such information is available, to take the necessary action, including disqualification, either partial or entire, and shall further, by such action, waive any right to challenge the determination of the Department as to its share, if any, pursuant to the privilege of receiving shared revenue under this part. Recommendation: As indicated on the application, it must be signed by the Mayor or Chairman of the Governing Body. I recommend that the Commission approve the Mayor to sign the application as required to assure that the City will benefit from state shared revenues in the upcoming yeaz. Summary Recommendation: All three problems mentioned above can be solved within the existing budget by purchasing refurbished components. Although there is some risk with purchasing refurbished components, I feel that the lazge savings, coupled with the ninety day warranty and their subsequent inclusion on our maintenance contract, minimizes the risk and allows us to address all three issues. Without the savings realized by purchasing refurbished equipment, the memory problems would have to be addressed and funded by other means. I recommend that these issues be resolved as soon as possible by purchasing refurbished components. Below is a schedule of the estimated costs and savings. Solution: Issue 1 can be addressed by purchasing a six line communications controller, (model #9404-2623) and three (3) two line adapters, (model #9404-2609 EIA 232/v.24). These components will add six lines of communication to the system for current use and future expansion, such as the possibility of the City Clerk's office having a dedicated line to the Property Appraiser's office. Based on my consultation with other users of similar equipment, I am recommending the purchase of refurbished components at a quoted cost of $585. The same equipment would cost $4,584, if purchased new. Acquisition of refurbished equipment would save the City $3,999, excluding installation. Atlantic Beach Code Section 2-336 (7) states that formal bid procedures aze not required when purchasing data processing equipment. Since components added to our existing IBM Black Box computer are warrantied ~ if IBM installs and maintains the equipment, I recommend purchasing the components from an IBM dealer. Once installed, these components will be added to our existing AS/400 maintenance agreement with IBM at no additional charge. The fiscal yeaz 1997/98 budget provides for $3,700 to purchase the items mentioned above. The approximate $3,000 of funds remaining after acquiring these items are available to provide the memory and capacity needs described below. Issues 2 and 3 can be solved with the purchase of one (1) 32 MG main storage, (model #9404-3172), and three (3) 1.96 GB Disk Drives, (model #9404-6606). The addition of the 32 MG of main memory will enhance the processing speed, (this is like RAM on a PC), increasing available main storage from 80 MG to 112 MG, or 40%. The addition of the three Disk Drives, (this is like a Hazd Drive on a PC), will increase disk capacity from ] 1.76 GB to 17.64 GB, or 50%. I anticipate this change to bring the system capacity below 50%. These components would be added to the existing configuration and located in the available open slots housed within the Black Box adjacent to the existing memory, as was originally planned. The combined cost of these components would be $8,072 if purchased new. I recommend purchasing refurbished components at a quoted cost of $1,620, excluding installation. This saves $6,452, and, as with the wmmunications components, they would be under warranty for ninety days and would also be added to the maintenance contract for no additional charge. Issue 1 As more users are needing to use the system from various remote locations, the demand for the two lines currently available has increased such that the sharing of these lines prohibits users to sign on at their convenience. One line is dedicated for our software support, which is typically used for hours at a time. The other line is shared between IBM, Auditors, Parks and Recreation and Public Works. Public Works has requested the addition of another location (1100 Sandpiper Lane) for requisition input and account inquiry. Issue 2 Response time has slowed dramatically due to an increased number of users and installation of an HTE software enhancement called "Menu Driver". Menu Driver allows users to build a daily menu of application and function access, tailored to their individual needs, providing for a more efficient transition from inquiry to transaction processing. For example, a cashier can quickly review a customer's account while processing a payment transaction, without having to back out of one screen to access another. This enhanced capability provides for increased productivity, however, without adequate available memory, performance will continue to diminish. Issue 3 Available memory is approaching maximum capacity. System status as of May 12, 1998 indicates we are at 75.45% of available memory. The recommended capacity threshold is 90%, at which time the IBM representative assures us the system will begin to have memory conflict problems when executing jobs. In April of 1997, the system status indicated we were at 60.88% percent of capacity. As you can see, the memory capacity has decreased by about ] 5% in one year, and by this time next year, we will probably reach our capacity threshold. The landscaping in front of the pond and the fountain were installed last week. Our new signs and landscape lighting aze on the way. You can imagine how excited we aze! The reason I am writing is to ask for your help with the aquatic weeds in the pond. As you know we had a neighborhood pond cleaning day last November to clean the weeds out of the pond so that our fountain could be installed. We were advised to get out as many of the weeds as possible and then the winter weather would kill back much of the remaining growth. Thanks to El Nino (everyone else is blaming it so I will too) the weeds have enjoyed a winter growing season and the fountain company is refusing to warranty the fountain because of the excessive weed coverage. I am hoping that the city will spray the pond for weeds at the same time the city ditches that run along the East and North sides of the pond are sprayed. I realize that this is an expensive proposition but the association has already paid $5,000 to improve the area and install the fountain. It would be a shame to have the fountain fail in a short time because the weeds can block the intake and bum out the motor. Thank you for your time. Andrea M. Pennell AGA President ~. Proposal: The City of Atlantic Beach is planning to use Byme Grant funding to initiate a community/recreation progam in Jordan Park. The City already owns a building at that location that has been dedicated to the Community Development Corporation (CDC), and the CDC is supportive of the idea and is happy to commit the use of the facility to this new project. The City of Atlantic Beach Parks and Recreation Department will organize and staffthe progarr~ and the Atlantic Beach Police Department will participate by providing matching funds for the gant through contraband/forfeiture funds. Initial Budget: To initiate this project, the following budgetary needs have been identified by staff: Personnel This progam will be staffed approximately five hours,/day or 20 hours/week during the school year. During the summer months, the staffing will be increased to 8 hours/day or 40 hours/week. Additional funding is anticipated for Christmas holidays, Spring Break, and other times. At the expected hourly rate of $7.00/hour, the total cost of the personnel from October 1998 through September 1999 is $9,730. Operating Expenses The type of equipment that will be needed initially will include educational supplies for homework and tutoring, VCRlTV for videotaped presentations, tables, chairs, and computer equipment (mostly support equipment for donated computers). Recreational equipment will be needed as well as general maintenance, supplies and first aid kits. Training may be necessary for staff members, and it is anticipated that educational opportunities at local colleges and museums may require some funding. For the first year of operation, the total for operating supplies is estimated at $8,510. Thomp~ri, Director of Public Safety Timmy Johnson, Parks and Recreation Director DATE: May 13, 1998 BACKGROUND: Ten (] 0) years ago, the Atlantic Beach Police Department identified the areas of the city with the highest level of drug activity, including illegal narcotics sales and drug related violent crime. These areas were primarily low income neighborhoods including the area around Donner Park, and the neighborhood around Jordan Park. Attempts to organize Neighborhood Watch Programs were unsuccessful, lazgely because of the alienation that the citizens felt from their neighbors and everyone else. Over the next ten (10) yeazs, Habitat homes were constructed, a Community Development Corporation was formed, Community Development Block Grant funds were dedicated, code enforcement became very active, and the Police Department focused resources on these neighborhoods. There were considerable improvements made in the quality of life for everyone in these affected areas. However, in the Donner Park area, changes were the most dramatic. In ] 992, a Community Center was initiated in Donner Park with the use of Byme Grant funding. The center started out very modest with a rented building and part-time staff. However, the program brought the children in the neighborhood together. Soon, the teenagers were helping with the program, and the parents started participating and contributing. Homework assistance and tutoring became available with incentives for increased school performance. Championship sports teams emerged. Summer camp was initiated with state funded meals, and senior citizens volunteered their time with kids. The Donner Center has recently had computers donated to it for Internet access and research. The City built a new facility in Donner Park that serves hundreds of children every week. This program has served its community well, but there are others in the City that have not been able to benefit from it. Across Mayport Road in the Jordan Park area, children and adults have remained detached from the Donner Center, largely due to the physical separation created by Mayport Road. The Jordan Park area has had the other initiatives noted above with the exception of a community center. Although the housing has improved and the neighborhood has benefited from park development, there is no supervised or organized community center. Replace damaged irrigation components which cannot be repaired with new functioning components by same manufacturer. 4.4 Tum system off during times of frequent heavy rains and high ground water levels. 4.5 Adjust system for more frequent watering in very dry times or drought conditions. 4.6 Water median planting at programmed intervals to maintain good color, health and sturdy growth of plant materials. Start water cycle at 3:00 AM and end at 6:00 AM on water cycle days. 4.7 Spot water locations of new planting as necessary. 51. CONTRACTOR will be responsible for safe conduct and use of equipment on job site. 5.2 All maintenance work must comply with FDOT requirements and specifications, including prescribed hours allowed for work to be performed, if any, and all safety requirements. 5.3 Highway lane closures should be avoided; however, in the event a lane closure becomes necessary, it must be approved by the Atlantic Beach Public Works Department with a minimum of 48 hours notice. Page 4 SHRUBS: Use a complete and approximately balanced fertilizer such as a 10-10-10. Apply at the rate of 1 to %: 16. of Nitrogen, per 1,000 square feet. Broadcast over the entire azea under the shrubs, four times per yeaz in March, June, September, and December. SMALL TREES: Small trees can be fertilized the same as shrubs. Use 2 %: lbs. of 8-8-8 or 3 ibs. of 6-6-6 per inch of tnuilc diameter. If a different analysis is used, apply an equivalent amount of Nitrogen Broadcast under and slightly beyond the entire canopy. Maintenance fertilization should be made when tree is dormant. If grass is growing under the canopy, it may be desirable to apply the fertilizer in 12" punch holes. 2.3 INSECTS AND DISEASE CONTROL FOR PLANTS. The CONTRACTOR is responsible for treatment of insects and diseases. The appropriate fungicide or insecticide will be applied in accordance with state and local regulations, and as weather and environmental conditions permit. Inspect plants weekly for infestation. Inspect for Leaf Miners weekly in Lantana beds. Any plants that die from insect or disease will be replaced at CONTRACTOR'S expense. 2.4 WEED CONTROL IN PLANT BEDS. A. The CONTRACTOR shall inspect and weed plant beds weekly for any weeds and errant grasses. B. Control weeds with use of herbicides, pre-emergents, and, preferably, by proper manual removal of weeds and their root systems. C. Remove noxious weeds common to the azea from planting azeas by mechanical means and dispose of properly. Do not leave uprooted weeds in plant beds. D. Apply herbicide in accordance with manufacturer's published instructions. E. Do not apply herbicide when wind speeds are greater than 2 m.p.h. The CONTRACTOR will replace any plants that are damaged by the use of herbicides at their expense. All chemicals shall be provide by the CONTRACTOR. 2.5 MULCH. The CONTRACTOR shall apply a 3" layer of pinestraw mulch around all palms, shrubs and groundcovers. This shall be performed during the month of Mazch each yeaz. Care shall be taken to avoid trampling and smothering of shrubs and groundcovers during mulch application. Apply pinestraw mulch only. Substitutes will not be allowed. Any other mulch applied will be removed at CONTRACTOR expense. 2.6 PALMS TREES. Are to be fertilize using 16-4-8 or 12-4-8 with minor elements. Apply 2/3 Lb of fertilizer per inch of palm trunk diameter four times per yeaz in Mazch, June, September, and December. Adjust rates if you use other analysis. Broadcast under the entire canopy. Contact Greenscape of Jacksonville for proper source for palm fertilizer. Page 3 1.6 DISEASE IN TURF. Turf damaged by disease and fungus shall be chemically treated to maintain all turf in a healthy and attractive condition. Any turf that is lost to disease or fungus will be replaced at the CONTRACTOR'S expense. 1.7 WEED CONTROL IN TURF. The CONTRACTOR shall apply as necessary according to environmental and seasonal conditions, chemicals to control weeds in turf. Caze shall be taken not to apply herbicides on newly mowed turf or turf under stress. Any turf or plants that aze lost to misapplied herbicides will be replaced at the CONTRACTOR'S expense. The CONTRACTOR is to be responsible for maintaining the proper licensing necessary to apply insecticides, fungicides, and herbicides or to subcontract these services to a properly licensed applicator and the CITY is to be provided with copies of the PEST CONTROL OPERATOR'S license and proof of liability insurance. All chemicals shall be provided by the CONTRACTOR. PART 2. SHRUB. GROUNDCOVER. AND BED MAINTENANCE 2.1 PRUNING. Plants and shrubs shall be pruned by the CONTRACTOR at the best time for flower bud development, foliage growth, and as the health of the plant may require. Shrubs shall not be clipped into balled or boxed forms unless such is required by the design; they will be pruned in accordance with the intended function of the plant in its present location, and as often as needed to maintain health and appeazance. Pruning is an art that must be under the supervision of a highly trained foreman. CONTRACTOR shall remove non-hedge growth from hedges and shrubs; such as Spanish moss, vines and grazses. Prune or head back plants in keeping with natural chazacter of plants. Do not uniformly sheaz plants. Prune Crape Myrtles once a yeaz in February in a natural tree form character according to Florida Nurseryman's Growers Association and Duva] County Extension Service guidelines. Establish radial branch orientation and remove narrow V-shaped forks, cross-over branching, and branches that rub against each other. Caze shall be taken that trees and shrubs are not scarred and that fences, buildings, signs, etc. are not damaged. 1.4 TURF FERTILIZATION. The CONTRACTOR shall apply fertilizers to all turf azeas using a complete fertilizer having a minimum 3:1:2 ratio such as 16:4:8, and containing a minimum of 25 per cent slow release nitrogen Fertilizer is to be applied at the rate of 1 lb. of nitrogen per 1,000 squaze feet and will be applied in March, May, July and September, or as determined by the city. Should any fertilization result in a streaked appearance due to misapplication, corrective applications will be made at the CONTRACTOR'S expense. The CONTRACTOR is required to provide proper training for employees who apply fertilizers. 1.5 INSECTS IN TURF. The CONTRACTOR is responsible for treatment of grass damaging insects on a preventative basis. An insecticide is to be applied to all turf azeas, a minimum of four times per season to reduce or eliminate the population of chinch bugs, mole crickets, sod webworms, army worms, and grass loopers. Uniforms shall be delivered to and picked up and returned to the following City location: City of Atlantic Beach Public Works Department 1200 Sandpiper Lane Atlantic Beach, Florida 32233 7. The vendor shall launder and repair or replace uniforms as applicable. If the cost of repairs is not included in the rental charge, it shall be stated separately. Costs for garment reported as missing shall also be quoted. 8. A City logo patch shall be sewn over the left pocket worded "CITY OF ATLA.yTIC BEACH ", and a patch sewn on the right side of the shirt indicating the employee's first name. 9. All employees shall be fitted by the vendor according to sleeve length for long sleeved shirts. Trousers shall be fitted for both waist size and trouser leg inseam. A copy of each individual's measurements shall be given to the Purchasing Department. Uniforms provided which do not fit properly according to the measurements will be returned to the vendor without obligation to the City. 10. Prices for all garments, other than jackets, shall be quoted on a weekly rental basis, including laundering and nor..al repairs. There shall be eleven (11) sets per employee to enable each person to receive five sets, turn in five sets, and have one to wear at the time of weekly turn-in. 11. Employees will turn in weekly for laundry and applicable repair those uniforms worn during the previous week. This will normally consist of five sets except when the employee has worked less than five days during the prior week. The uniforms turned in during the previous week shall be returned in exchange. A consistent day of the week shall be scheduled for pickup and delivery. Pickups and deliveries shall be made weekly on the designated day between 7:30 AM and 3:00 PY., 2fonday through Friday, except for legal holidays for which prescheduled alternate arrangezents shall be made. The Purchasing Agent contacted eight local firms about the upcoming bid, but only two bidders responded: Aramazk Uniform Services, Inc. and RUS of Lakeland, Inc. The Aramazk bid was incorrect because the unit prices for items 1 - 10 were corrected by a letter received on May 19, 1998, five days after the bid opening. The RUS bid did not include any references and bidders were required to submit these references with their bid package. Therefore, we had no responsive bidders. The contractor shall provide all necessary personnel, equipment and chemicals and shall be required to control the growth of aquatic plants in each specified body of water for one year following the first treatment. 3. For each ditch or lagoon, the contractor shall recommend a program to include initial treatment(s) and subsequent maintenance treatments and submit same to the Director of Public Works for review and approval. Recocenendations shall include chemicals to be used; technique and rate of application and verification of same to the Director of Public Works; and time and place of initial application(s). 4. After initial control has been established, the contractor shall, on his own, inspect each body of water at least one time per month and spot treat as necessary to maintain growth control. The City shall be notified each time a body of water has been inspected and advised what action, if any, was taken. 5. In addition to these routine inspections/treatments, [he contractor shall respond in a timely manner to requirements for additional treatments if the City deems them necessary. `~THOD OF OPERATION - An Inverted Purchase Order shall be issued through which payment for all treatments will be processed. Receipt by the contractor of this purchase order will be the Notice to Proceed. The first treatment shall be accomplished within ten (10) days following notification of award. INGRESS/EGRESS - The City shall arrange for and designate ingress/egress points to work areas and the contractor shall use only these points. LICENSE(S) AND PEPMITS - The contractor shall possess all required licenses and permits to comply with City, Slate and Federal Regulations. REPORTS - The contractor shall provide the City with a report of operations including location, application technique, acreage of treatments, species treated, chemical and rates applied and any other related information or recommendations that may be of importance to the City in planning future aquatic weed control. The report will be supplied to the Director of ?ublic Works within one month after each treatment is complete. Bids were submitted by Lake Doctors and Vortex Biological Services, Inc. Lake Doctor: No Bid due to Bond and Weed eating requirements. Vortex Biological Services: $31,494.00 Yeazly. However, they did not submit a Bid Bond. If we waive the Bid Bond requirement and go out to re-bid, Lake Doctors would not bid since they do not do weed-eating. RECOMMENDATION: City Staff has spoken to Vortex Biological Services, Inc. regarding an extension of their current contract for one year at the current rate of $31,494.00, which is exactly what they bid on April 8, 1998, and they have agreed. Grunewald, Safety Officer May 21, 1998 Background: Process Safety Management (PSM) and Risk Management Plan (RMP) are intended to prevent accidents and minimize the consequences to the public and employees from releases of toxic or flammable substances. The rule applies to all stationary sources that store or "process" more than a threshold quantity of a regulated substance at any one time. Chlorine and sulfur dioxide are substances regulated by RMP and used by the City of Atlantic Beach in quantities in excess of 2,500 and 5,000 pounds. Quantities aze regulated by PSM in excess of 1,500 and 1,000 pounds respectively. The Florida Division of Safety has mandated that the City of Atlantic Beach meet all of the requirements of the above listed OSHA Standards.
