Fee to Establish Service After Cut-off If water service is turned off, either because of delinquency or upon the request of the consumer, for any purpose, a cut-on fee of eex fifteen dollars E81B.BB~ (515.00) shall be charged and paid when water is turned on. ARTICLE 3 KASTEWATER SYSTEM DIVISION 7 SEWER USER CLASSIFICATION RATES AND CHARGES Section 22-174. Delinquent Bills If any bill for monthly sewer, water or garbage service shall remain due and unpaid on and after the thirtieth day from the billing date, dated on the bill, a penalty of five dollars (;5.00) shall be imposed and added to the bill. The City' shall mail at that time a delinquent late notice to the user stating that if the bill remains unpaid for a period of ten (10) additional days, then the City shall discontinue the service. If the bill remains unpaid for a period of ten (30) additional days, then all services shall be discontinued and shall not be reconnected until all past due charges have been fully paid together with a turn-off and turn-on charge of 6ea fifteen dollars {s~s.ee) (51~5.00~. Ths Flw Isolation Study is to be per£oxmed in one maMole section at a tine, plugging all incoming lines from upstream manholes. Utilizing a '. standard 90' Y-notch rein, the flow over the weir 1s to be read in '~ gallons per day. Simultaneously rith installing the weir in the lice, ', the successful bidder is to perfon a manhole inspettion and at intervals during inspection, the lwsl over the weir is checked and the quamtiry of flow noted. This muss be done until tw successive readings - approzimtely three to five minutes apart o£ the sae magnitude are recorded. this will allot a stabilization time far the flow over the wit and ensure a uauate measurement of infiltration. After obtaining the weir reading, the successful bidder will then plug all incoming lines into the manhole, return to the upatrea manhole, and rmmove the plugs installed there, and then proceed to the next downstrea manhole to repeat the procedure of manhole inspection aM rein readings. This process is to be repeated until all of the areas designated by the enclosed map have bern flow isolated, and manhole inspections have been psrfazmed on all designated manholes in she area. Mayor Rowell called the special meeting to order ac 5:00 p.m. for the purpose of hearing a report from the Committee on cleaning, televising, and sealing of a portion of the City's sever collection system. Those present were Mayor William S. Howell, Commissioners Roherc B. Cook, Sr. and Glean A. Edwards; City Manager Richard C. Fellows, and City Clerk Adelaide R. Tucker. Commissioner Edvarde reported the committee met on three occasions to discuss [he proposals of the two low bidders, Altafr Maintenance Services, Inc. of Longwood, Florida, and Southern Line Cleaning of Sanford, Florida. During the discussions it became apparent chat a floe monitoring study concluded September, 1983 under the auspices of Robert Bates and Associates, provided the basic ground work Eor the next step in isolating the problem areas in the collection system which is a flow isolation study. This would be more appropriate at this time than the more expensive cleaning and televising. After a full discussion of the merits of [he various alternatives, the committee voted unanimously to recommend to the City Commission chat all bids be rejected and the committee be authorized to seek sealed proposals for a floe isolacion study utilizing the flaw monitoring report of September, 1983 as the basis for more precisely defining the real trouble spo to in the City's sever collection system. Commissioner Gulliford agreed and said the Delegation should take [be lead and a meeting of all the interested parties and the Delegation should be sec. Commission agreed and in- structed Mayor Novell [o proceed along those lines and report back. Nr. Fellows reported back when the Ci[y employed Mr. Robert Stubbs [o do a study on establishing sever rates which vas subsequently approved by DER, members of the Commission would recall that Ordinance did no[ include a provision that [he City could put a surcharge on outside the city. Through Nr. Nullis's efforts, the matter vas negotiated out, but during that interim period the Ordinance vas in a static position. Once [he surcharge vas approved, [he City Commfssio, passed the Ordinance in order to begin drawing down on the grant. After passing the Ordinance and fine tuning on the commercial aspect, he found adj uatmenta needed to be made so that [he commercial charges are equitable in [hat [hose large firms pay a large amount and the small firma pay a small amount. Ne stated if the Commission had no objection he did not intend to implement the Ordinance on January 1,1986. He offered a bid from St. Paull for additional coverage for a total limit of $1,000,000 with a $1,000 deductible for a coat of $2,185. We are enclosing a copy of it so that you may be up to date on the status of cases before the City. o A [o®unication from the Chairnan of the Advisory Planning board is enclosed with recommendations on actions taken by the Board at their meeting of December 17. This is in accord with the new Ordinance recently adopted. The City Commission should be keeping in mind it will be necessary to appoint one more member to the Advisory Planning Board to replace Mr. Morris. The term would be from January 1, 1986 through December 31, 1989. o In regards to the appointment of new members, the City Commission should be aware of the need for appointment of two members to the Code Enforcement Board. The terms of Fred Mills and Willie Davis expired October 1, 1985, and the new appointments would serve for three years with their terms ending on October 1, 1968. o David Baker has brought us a special Warranty Deed for the extension of Hibiscus Street 235 feet south of its present ending. This is in accord with his agreement with the City at a previous meeting. If in order, the City should accept the Warranty Deed and the right of ray. o We have concluded an arrangment with Don Ford to continue as full time Building Inspector with the City of Atlantic Beach. He has replaced John Widdows xho resigned. Frank Kezbex had been filling the position temporarily, but re have been much impressed with Don Ford's ability and we feel he represents the City of Atlantic Beach very cell. 0 Two agreements will be up for consideration by the Lity Commission at its meeting Monday night. One is between the City and Devtronics. The Department of Environmental Regulation has told Devtronics they must either hook to the City of Atlantic Beach treatment syst®, or build their own plant. We have worked out the details and Mr. Mullis has prepared an agreement which should protect the Lity. The second is an agreement between the City and BSM Partners on sewer source at Lakeside Shopping Center off Mayport Road. P WRIUA J223J TF.LF PHONE I9a12~42295 At the request of the City Collmli ssion, the committee met to examine and evaluate the bids submitted for water project improvements to the Navy off-base housing project and a proposed flow isolation study of a section of the City's sewer collection system. It is the recommendation of the committee the City Commission authorize the award of a contract to the low bidder, Jax Utilities Management, in the amount of $37,356.29 for the water improvement program for the Navy off-base housing project. TI]e committee also recommends award of a contract to Griner's A-One Pipeline Services of Orlando, the low bidder for the flow isolation study, in the amount of $3,600.00. In regards to the construction administration on the Westside force main recently awarded to MacDonald Corporation, the committee would like to recoowend the use of the desigr. engineer on the project, Waitz and Frye, for contract and construction administration. For a lump sum of 62,500.00 they will prepare the necessary contracts, conduct the pre-construction conference, and prepare periodic construction progress reports, and authorize payments to [he contractor.
