At the present time there is a contirk-_ rigid slab from the brick face of the adjacent structure up to and against the tub . 6. Since placing the tub and during the placement of concrete there has been no protection provided the tub . Consequently concrete has been splashed onto the surface, has hardened and must be carefully removed and the surface polished to restore the original finish. In addition the tub is now filled with a mixture of sand, debris, twigs, leaves, loose wire and a light fixture. I strongly suggest that the condition noted in reference to the tub, piping and associated mechanical equipment be corrected at the earliest possible time and that the tub surface be cleaned and polished to a finish equal to that existing when the unit was delivered. I. 7. Concrete Work: a. The placement and finishing of the concrete slab was not performed by craftsmen of the trade, this is quite evident to the most casual observer. Some of the most obvious errors were; 1 . There was inadequate fill material available on site to rise the area to the proper elevation. Additional fill material was delivered late in the day after the pour was made. In an effort to overcome this lack of material only the area inside the forms was filled and compacted. The result was that the stakes driven to hold the forms in place were driven into material that had not been compacted and subsequently the forms shifted. When this happened the top of the forms normally used by the finishing mason for estab- lishing slope and finished elevation were no longer reliable as a reference. The result was a finished surface elevation that in some cases sloped in the wrong direction, did not slope at all or in other cases sloped in excess of that specified. The form material was inadequate, crooked, and the forms were not set at a level compatable with the required slope. At the NW corner the form was not continuous allowing the concrete to run through the form. An inspection of the site immediately prior to concrete placement indicated that the reinforcing steel was placed directly on the plastic film covering the fill , unless some type fixture was placed under the steel to raise it to aproximately the midpoint of the slab prior to placement of the concrete, the reinforcing effect of the steel will be nearly zero. If there were any masons tools on the site they were not used, examples; - The tub was to be set aprox. 3/4 inch above the finished slab elevation. On two sides of the tub concrete was allowed to rise to the bottom of the lip, a single pass with a trowel would have removed this material during the finishing process. - The edges of the slab were not finished with an edging tool as is customary in the trade, the result is the slab will have a sharp edge that will spall and break after the forms are removed. - At the point where the new work butts the existing slab the joint was not sufficiently worked with a finishing trowel to effect a continuous surface, in fact the two surfaces are not even at the same elevation, in some places this elevation of the new work is as much as 3/4 inch lower than the existing slab . The concrete was not worked sufficiently to remove the obvious surface voids at the corners of the forms. This condition prompts one to be concerned as to the existence of serious voids in the remainder of the pour. During our initial discussion relative to construction of the slab I provided a list of items relative to performing this task. (copy attached) You were in agreement with the content of this list,signed it, and stated that you saw no problem in performing the work as specified. One of the points discussed was the use of reinforcing steel mesh to prevent cracking of the slab. The subsequent imprinting of the surface with unnecessary, ineffective, unsightly joints every few feet is a very objectionable, unsightly and unwar- ranted departure from normal concrete finishing standards. Prior to the placement of concrete my wife reminded your Foreman that the concrete truck was not to be allowed to back onto the driveway, this was ignored and the driveway has been extensively damaged. In conclusion ; Your assertion at this time that "this is the best that we can do " ; I find to be completely unaccep- table . As you departed on the morning of 15 November you had made two distinct offers to remedy the situation, I shall repeat them as a reminder: a. You would have the slab completely removed, and we could have it replaced by our own contractor . or b . You would have the slab, tub and all associated equipment removed and return our money. At that time you indicated that you would put your offer in writing, therefore we are requesting reciept of your response not later than 21 November 1988. ere 1y a Ave SKETCH AVE-09-88 1665 PARK TERRACE W. ATLANTIC BEACH,FL The following notes are part of the sketch identified above. 1 . The sketch identifies the existing concrete patio , underground sprinkle lines and the new addition of aproximately 347 SF to be constructed. 2. The new slab is to be 4 inches thick, 2500 PSI concrete poured over suitably compacted fill to an elevation and finish matching the existing. New work will be sloped 1/8 inch per foot from the existing slab and/ or tub to the outside edge. 3. The new slab will be attached to the existing slab by drilling into the edge of the slab a minimum of 8 inches at 18 inch intervals, no. 3 bar will be inserted into these holes and extend into the new work a minimum of .12 inches or overlapping the reinforcing steel of the new , work by 8 inches. 4. The new work will be reinforced using 6 by 6 by # 10 welded wire reinforcing, placed to provide a minimum of 1 1/2 inches of concrete covering the wire. 5. The new slab will be poured over 6 mil polyethelene film laid directly on the compacted fill . 6. After completion and removal of forms the area adjacent to the new work will be filled with suitable soil and backsloped 1 on 5 from top of slab to the existing ground elevation. 7. A code approved electrical circuit and bonding system will be run from the existing electrical panel located in the adjacent garage , into the attic and overhead to a location in the roof overhang, down the outside wall , thence underground to emerge at the supporting slab for the pump and filter.
