L. is pleased to present this threshold Inspection Monthly Report No. 2 for the LA Fitness Atlantic Beach project located on 985 Atlantic Boulevard in Jacksonville, Florida. Our Services on this project were performed in general accordance with available project documents prepared by Cuhaci & Peterson Architects Engineers Planners, dated January 27 and June 5, 2012. In general the work activities performed during the period of June 1 through June 30, 2012 includes Cast In-Place concrete inspections for slab on grade, piles, CMU walls, and pile caps. We are retained to perform threshold inspection services on this project on "as-needed" basis (i.e. our representatives are not present on site at all times during the construction activities). The threshold inspections performed by our representatives typically include periodic observations of construction activities for substantial compliance with the project documents. This Letter addresses Threshold inspection Reports Nos. 3 through 15 performed during the period of June 1 through June 30, 2012. The Construction Materials Testing (CMT) reports were reviewed and no deficiencies were noted. The reports include concrete and grout testing and Auger Cast In-Place (ACIP) piles monitoring. These reports are available, if requested. Relevant items pertaining to the structure that have been inspected by our personnel and appear to deviate from available contract documents that have been approved by the Engineer of Record(EOR)are listed in the threshold inspection reports, and in the Non-Compliance Record. The non-compliant items will remain on the Non-Compliance Record until written response is submitted by the EOR. Responses properly issued by the EOR and received by this office will be incorporated into that period's Threshold Progress Report.
