Construction Site Management Plan Compliance
A construction site management plan conforming to Atlantic Beach City Code Sec 6-18
has been approved as a part of this building permit. The Construction site management
plan was approved based upon the following information.
1. Parking plan — parking plan showing how site will be accessed and all onsite
and abutting street parking areas.
2.
3. Location of construction trailers, loading/unloading area and material storage
area.
4. Location of chemical toilet area.(chemical toilets must be kept out of City
right-of-way and not further than 15 feet from structure under construction)
5. Location of dumpster. Dumpster must be from an approved waste company
(in accordance with Chapter 16 City Code) as of 2009 the permitted
dumpsters are Advanced Disposal, Realco Recycling, and
Shappells. Dumpsters will have tarp covers or rigid covers on windy days.
Dumpsters must be removed prior to issuance of Certificate of Occupancy.
6. Traffic control plan, showing access with dimensions, area to be stabilized,
narrative on phasing of construction with adequate parking and delivery of
materials.
7. Site cleanliness. Contractor must have the entire construction site cleaned by
Friday of each week. This means removal of scrap lumber, concrete remnants
and other such construction debris including cans, metal, plastic and paper.
8. Erosion and Sediment Control. Contractor must maintain all elements of the
approved Erosion & Sediment Control Plan (silt fence, catch basin filters, etc.)
until sod or other stabilization has been placed and approved by Public Works.
9. Other activities, where special conditions are identified by the Building
Official.
Failure to comply with the Construction Site Management
Ordinance may result in a Stop Work Order being issued in
accordance with City Code Sec. 6-17 (3)

without limitation the power to pay for medical, dental and surgical care, living

assistance, including without limitation Social Security, Medicare, Medicaid,

In addition, I specifically authorize my attorney-in-fact to make gifts of my

objective of preserving the largest amount of my property for my family as a whole. The gifts

8821 (Tax Information Authorization), or comparable authorization, appointing a qualified

foreign taxing authority with respect to the types of taxes and years referred to above, and to
specify on said authorization said types of taxes and years; to receive from or inspect confi-
dential informaiion in any office of the Internal Revenue Service or state, local or foreign tax
authority; to receive and deposit, in any one of my bank accounts, or those of any revocable trust
of mine, checks in payment of any refund of federal, state, local or foreign taxes, penalties and
interest; to pay by check drawn on any bank account of mine or of any revocable trust of mine
and have accounts to permit my attorney-in-fact to draw checks for payment of said items; to
execute waivers (and offers of waivers) of restrictions on assessment or collection of deficiencies
in taxes and waivers of notice of disallowance of a claim for credit or refund; to execute any
requests for extension of time and consents extending the statutory period for assessment or col-
lection of such taxes; to execute petitions contesting taxes; to establish new residency and
domicile; to execute offers in compromise and closing Agreements under Section 7121 or
comparable provisions of the Internal Revenue Code or any federal, state, local or foreign tax
statutes or regulations; to delegate authority or to substitute another representative for any one
previously appointed by me or my attorney-in-fact; and to receive copies of all notices and other
written communications involving my federal, state, local or foreign taxes at such address as my
attorney-in-fact may designate.
It may be necessary for my attorney-in-fact to have access to my medical records
to establish whether medical bills are valid and appropriate or for other purposes. I grant to my
attorney-in-fact the authority and power to serve as my personal representative for all purposes
of the Health Insurance Portability and Accountability Act of 1996, the regulations in 45 C.F.R.
Sec. 160 et seq., and any other applicable federal, state or local laws or regulations
(collectively "HIPAA"), including the authority. to request, receive, obtain and review, and be
granted full and unlimited access to, and consent to the disclosure of complete unredacted copies
of any and all health, medical and financial information and any information or records referred
to in 45 C.F.R. Sec. 164.501 and regulated by the Standards for Privacy of Individually
Identifiable Health Information found in 65 Fed. Reg. 82462 as protected private records or
otherwise covered under HIPAA. I understand that health and medical records can include
information relating to subjects such as sexually transmitted diseases, acquired immuno-
deficiency syndrome (AIDS), AIDS-related complex (ARC) and human immunodeficiency virus
(HIS, behavioral or mental health services, and treatment for alcohol or drug abuse or
addiction. I understand that I may have access to or receive an accounting of the information to
be used or disclosed as provided in 45 C.F.R. Sec. 164.524 et seq. I further understand that
authorizing the disclosure of this health information is voluntary and that I can refuse to sign this
authorization. I further understand that any disclosure of this information carries with it the
potential for an unauthorized further disclosure of this information by third parties and that such
further disclosure may not be protected under HIPAA. In order to induce the disclosing party to
disclose the aforesaid private and/or protected confidential information, I forever release and

Jacksonville Beach, Florida 32250, Grantor, and GILMAR PARTNERS LTD, whose post office
address is 1251 Beach Blvd., Jacksonville Beach, Florida 32250, Grantee:
Witnesseth, that said Grantor, for and in consideration of the sum TEN AND NO/100 DOLLARS
($10.00) and other good and valuable considerations to said Grantor in hand paid by said Grantee,. the
receipt whereof is hereby acknowledged, has granted, bargained, and sold to the said Grantee, and
Grantees heirs and assigns forever, the following described land, situate, lying and being in Duval
County, Florida to wit:

of West 11th Street (a 50.00 foot right-of-way as now established); and run North 69033'52" West
along said northerly right-of-way line of West 11th Street, a distance of 154.85 feet to its intersection
with the westerly line of aforementioned Lot 3, Block 44; run thence North 20126'58" East along
said westerly line and along the westerly line of said Lot 3, Block 45, a distance of 164.00 feet; run
thence South 69133152" East, a distance of 154.82 feet to a point lying on the aforementioned
westerly right-of-way line of Mayport Road; run thence South 20028'35" West along said westerly
right-of-way line, a distance of 154.00 feet to the Point of Beginning.

Subject to taxes for the current year, covenants, restrictions and easements of record, if any.
And the Grantor fully warrants the title to said land, and will defend the same against the lawful claims of
