Please be advised, this letter is to inform you that permit #13-3419, 1500 W. Park
Terrace, failed an inspection on 1/02/14 and 1/08/14 for the following reasons:
"Strapping at beam at northwest first floor porch still not nailed off."
Failing two inspections for the same issues results in a reinspection fee of$55.00. This
must be paid in full before scheduling a reinspection.
Please correct the issue and call the inspection line at(904)247-5814 to reschedule.
