If available, an Agency website that fulfills this transfer requirement may be used in lieu of the form. 11. The permittee shall notify the District in writing of changes required by any other regulatory District that require changes to the permitted activity, and any required modification of this permit must be obtained prior to implementing the changes. 12.This permit does not: 1. Convey to the permittee any property rights or privileges, or any other rights or privileges other than t"rose specified herein or in Chapter 62-330, F.A.C.; 2. Convey to the permittee or create in the permittee any interest in real property; 3. Relieve the permittee from the need to obtain and comply with any other required federal, state, and loc:ai authorization, law, rule, or ordinance; or 4. Authorize any entrance upon or work on property that is not owned, held in easement, or control�ed by the permittee. 13. Prior to conducting any activities on state-owned submerged lands or other lands of the state, title to which is vested in the Board of Trustees of the Internal Improvement Trust Fund, the permittee must rece ve all necessary approvals and authorizations under Chapters 253 and 258, F.S. Written authorization that requires formal execution by the Board of Trustees of the IntE!n al Improvement Trust Fund shall not be considered received until it has been fully executed. 14. The permittee shall hold and gave the District harmless from any and all damages, claims, or liabilities that may arise by reason of the construction, alteration, operation, maintenance, removal, aband mment or use of any project authorized by the permit. 15. The permittee shall notify the District in writing: 1. Immediately if any previously submitted information is discovered to be inaccurate; and 2. Within 30 days of any Conveyance or division of ownership or control of the property or the system, other than conveyance via a long-term lease, and the new owner shall request transfer of the permit in accordance with Rule 62-330.340, F.A.C. This does not apply to the sale of lots or units in residential or commercial subdivisions or condominiums where the stormwater management system has been completed and cc nverted to the operation phase. 16. Upon reasonable notice to the permittee, District staff with proper identification shall have permission to enter, inspect, sample and test the project or activities to ensure conformity with the plans and specifications authorized in the permit. 17. If any prehistoric or historic a:-: facts, such as pottery or ceramics, stone tools or metal implements, dugout canoes, any other physical remains that could be associated with Native American cultures, or early colonial or American settlement are encountered at any time within the project site area, work involving subsurface disturbance in the immediate vicinity of such discoveries shall cease. The permittee or other designee shall contact the Florida Department of State, Civision of Historical Resources, Compliance and Review Section, at (850) 245-6333 0t (800) 847-7278, as well as the appropriate permitting agency office. Such subsurface wor�::3hall not resume without verbal or written authorization from the Division of Historical Resources. If unmarked human remains are encountered, all work shall stop immediately and ratification shall be provided in accordance with Section 872.05, F.S. 18.Any delineation of the extent: of a wetland or other surface water submitted as part of the permit application, including :dans or other supporting documentation, shall not be considered binding unless a specific condition of this permit or a formal determination under Rule 62-330.201, F.A , provides otherwise. 19. The permittee shall provide :-outine maintenance of all components of the stormwater management system to rem;;ve trapped sediments and debris. Removed materials shall be disposed of in a landfill or other uplands in a manner that does not require a permit under Chapter 62-330, F.A.C., or cause violations of state water quality standards. 20. This permit is issued based on the applicant's submitted information that reasonably demonstrates that adverse water resource -related impacts will not be caused by the completed permit activity. If my adverse impacts result, the District will require the permittee to eliminate the ca.is.e, obtain any necessary permit modification, and take any necessary corrective actions• to resolve the adverse impacts. 21. A Recorded Notice of Environmental Resource Permit may be recorded in the county public records in accordance with Rule 62-330.090(7), F.A.C. Such notice is not an encumbrance upon the grope_ rcy. 22. No work is authorized in, on or over wetlands or surface waters. 23. The operation and maintenance entity shall inspect the stormwater or surface water management system once within two years after the completion of construction and every two years thereafter to determine if the system is functioning as designed and permitted. The operation and maintenance entity must maintain a record of each required inspection, including the date of the inspection, the name, address, and telephone number of the inspector, and whether the system was functioning as designed and permitted, and make such record available for inspection upon request by the District during normal business hours. If at any time the system is not functioning as designed and permitted, then within 14 days the entity shall submit an Exceptions Report to the District, on form number 40C- 42.900(6), Exceptions Report for Stormwater Management Systems Out of Compliance. USGS Soil Survey for Duval County — map and soil description Project Narrative: Introduction — The proposed project (1.27 acres) is to construct a new greenhouse building and parking lot. The new construction will require retention and treatment of the stormwater runoff to meet SJRWMD and City of Atlantic Beach (COAB) requirements. Stormwater Treatment & Attenuation — The St Johns River Water Management (WMD) requires stormwater treatment and attenuation to be in accordance with Chapter 40C-42 FAC and the COAB will accept the conditions of the SJRWMD permit criteria. The eventual drainage outfall for the stormwater management facility (SMF) will be into an existing COAB roadside swale which flows west along West 1St Street and eventually to the Intracoastal Waterway. The peak runoff for predevelopment and postdevelopment conditions was calculated using the SCS method. A summary of the routing results is provided herein with the ICPR input/output data. The retention pond for this project has been over -sized for a maximum of 33, 000 sf of impervious area to accommodate possible future expansion of the greenhouse operations. Existing Conditions - The project previously had a warehouse building and two habitable structures. The site is currently vacant and most vertical structures and concrete pads have been removed. There are several trees remaining on the site that will be preserved to the extent possible. The existing site runoff generally flows from the east toward the northwest and southwest. There is no visual evidence of onsite or adjacent offsite flooding. Tailwater Conditions — The tailwater level (Boundary conditions) assumptions used in the stormwater model are based on the existing swale on West 1St Street flowing full at approximately 15" deep. Soil & Erosion Control — The engineering plans and notes indicate the requirement for placement of silt fencing and hay -bale. barriers to prevent the discharge of turbid waters. Additionally, there are notes in the plans that direct the site contractor to sod, seed or mulch all disturbed areas so that a permanent vegetative cover exists over all pervious surfaces. Wind-borne dust control shall also be monitored by the Contractor daily to minimize adverse impacts to neighboring owners. Dewatering — Dewatering may be required to construct the utility lines and stormwater treatment facilities. Discharged water from this process will be impounded in a settling basin, located near the center of the site, or routed through a gravel settling chamber prior to discharge from the site, or by such methods as may be determined by the site contractor. All required testing of the groundwater discharge will be performed by the site contractor. The rate of dewatering shall not exceed the consumptive use permit threshold. Operation and Maintenance Entity (OME) — The OME shall be the Property Owner, Atlantic Beach Urban Farms, LLC or assigned designee by official transfer. The maintenance of the stormwater treatment system shall consist of maintaining a permanent vegetative cover over all pervious surfaces, routine mowing and bagging of the debris, periodic inspection and cleaning of the pond banks and outflow control structure to remove debris and sediment. Construction Sequence — The project will be built in this general order; 1. Survey stake out of clearing and demolition limits 2. Construct silt fences, tree protection barricades and erosion control measures 3. Clearing of development areas 4. Rough grading of site, construct retention basin and parking lot 5. Underground utility installation and new building construction 6. Fine grading for pavement, swales and landscaping 7. Final paving and landscaping 8. Final inspections and approvals Construction Traffic — Construction traffic shall enter and leave the site from the Atlantic Boulevard or West 1" Street frontage. All public road surfaces adjacent to the site shall be swept or cleaned as needed to prevent the accumulation of sediment and debris on existing roads. Stockpile Areas — Stockpile areas shall be placed at least 10 feet away from any areas where runoff from the stockpile could leave the project development area. Extensive silt fencing shall be constructed around any piles that may have significant sloughing of its banks during a storm event. As an exception, the layer of silty fine sand at the location of Boring B4 extended to the boring termination depth of 15 feet. Very loose to very firm fine sand (SP) and fine sand with silt (SP -SM) were typically encountered below the silty fine sand layer and extended to the boring tennination depths of 15 to 25 feet. As an exception to the above, the layer of silty fine sand was not encountered at the location of Boring P2. A layer of roots (PT) was encountered between the approximate depths of 18 and 21 feet at the location of Boring Pl. Boring B4 encountered concrete debris between the approximate depths of 1 and 2 feet at the location of Boring A2. The auger borings (A1 through A6) generally encountered fine sand (SP) and fine sand with silt (SP -SM) throughout the 6 -foot exploration depths. As an exception, a layer of charred debris was encountered between the approximate depths of 2 and 3 feet at the location of Boring A2. Approximately 3 to 12 inches of topsoil and surficial rubble was present at all boring locations. The thickness of the topsoil will vary throughout the site. t!�.2 .;`Groundwater Level The groundwater level was measured at the boring locations, subsequent to boring `- completion, at depths varying between 3.0 and 4.0 feet below existing grade. The depth of the groundwater level encountered at each boring location is presented on the Generalized Soil Profiles and the Test Boring Records. The groundwater table will fluctuate depending on seasonal variations, adjacent construction, surface water runoff, etc. Based on the results of the soil borings, and review of available published literature, we estimate the seasonal high groundwater level at approximately 18 inches above the levels measured durinQ this exploration. Should rainfall intensity xceed normal quantities, or should other variables that affect the groundwater level be altered, the groundwater profile at the site could change significantly. 5.0 BUILDING AREA RECOMMENDATIONS 5.1 General The following recommendations are made based upon a review of the attached soil test data, our understanding of the proposed construction, and experience with similar projects and subsurface conditions.
