PERMIT APPLICATION FOR DRIVEWAY
Your permit application has been denied by the Public Works Department for the reasons listed
below. Please submit this information at your earliest convenience in order that we may
approve your application. If you have any questions, please contact Doug Layton, Public Works

PUBLIC WORKS CORRECTION ITEMS:
(Submit the following information to the Public Works Department)
**Provide table of impervious surface calculations for entire lot (existing and post
construction).
A. Cannot determine existing drive size
B. Provide calculations to support 779 SF final drive size
**Provide erosion and sediment control plans with installation details and maintenance
schedule. Provide drainage plans showing site topography (flow arrows, etc.)
**Provide a pre-construction topographic survey prepared by a Florida Licensed Professional
Land Surveyor, showing 1' contours, if on-site storage is required.
**Section 24-66(b) of the Land Development Regulations requires on-site storage for increased
run-off if adding 400 SF or more impervious surface. Provide Delta volume calculations and on-
site retention required per Section 24-66(b). (See attached information sheet.) You are
showing 400 SF+ increase but no storage.
**You may wish to utilize pervious pavers to be below 400 SF impervious threshold. If so,
submit catalog detail of proposed paver.
PUBLIC WORKS CONDITIONS OF APPROVAL:
(The following comments will be printed on your permit as Conditions of Approval)
*All concrete driveway aprons must be 5"thick, 4000 psi, with fibermesh from the edge of
pavement to the property line. Reinforcing rods or mesh area not allowed in the right-of-way.
*Full right-of-way restoration, including sod, is required.
*Roll off container company must be on City approved list and container cannot be placed on
City Right-of-Way. (Approved: Advanced Disposal, Realco, Republic Services, Shapelle's and
Waste Pro.)
*Full erosion control measures must be installed and approved prior to beginning any earth
disturbing activities. Contact Public Works (247-5834) for Erosion and Sediment Control
Inspection prior to start of construction.
