Included in this proposal is our understanding of the project, our proposed scope of services, fee quotation, schedule, and authorization procedures. INTRODUCTION On July 21, 201.5, an FPI representative -6sited the subject site to observe and determine a stabilization program.The referenced site is a single story concrete block structure constructed on a slab on grade. During our inspection the area of concern was pointed out to be the west comer of the structure where stair step cracking was noted in the exterior block on the northwest and southwest sides. Cracking and separation was also noted at the front porch, underneath the washer and dryer. Handheld static cone penetrometer testing revealed loose to medium dense soils to 4+feet below grade and the absence of a sufficient footer system. SCOPE OF SERVICES Based on our inspection,we recommend the following stabilization program. 5 model 250 steel push piers will be installed on the exterior perimeter of the structure, on approximately 6' centers. Two steel spreader beams will be installed for additional support/to spread the load of the pier at the two locations underneath the front porch slab. &' seamless gutters will also be installed to get moisture away from the foundation of the structure and mitigate the risk of future settlement. A laser level control system will be established and monitored during stabilization operations.Detection sensors will alarm peering technicians when heave or movement is detected. FPI will contact Sunshine State ONE CALL to have all underground utilities located as required by law. LIMITATIONS AND WARRANTY In performing our work, we will use personnel experienced and qualified in the various phases of your foundation stabilization, and will perform our services in strict accordance with industry standards and manufactures recommendations. We will attempt to level the structure back tows close to original condition as possible. Due to the nature of this type of repair.there is a possibility that any cracks or stress points created by the settlement may become apparent during any lifting or stabilizing procedure (usually minor). We will not be responsible for any cosmetic repairs_No liability shall be accepted by FPI for conditions which exist beyond our immediate control or knowledge. This shall include hidden or unknown condition and Acts of God. Some unforeseen conditions can arise including; hard mock, tree roots, buried trash, unstable soil collapses, etc. if these conditions are encountered and delay FPI's ability to perform timely, additional charges shall be evaluated and agreed upon byQth the client and FPI before continuing the project repair. Per our conversation, the piers at this specfic site need to be increased to a maximum spacing of 8 feet. Using a spacing of 8 ft the new pier loads are the following SP1 = 7520#( previously 5640#) SP2 = 5280#( previously 3960#) The new loads are still under the allowable load of 26,300 lbs ( Model 300 Steel pier).
