The following is a update on the CO inspections from Public Works.
317 East Coast This CO has several problems. I have talked to Jack Ossie and he is aware of these items.
1. Rocks need to be removed from the ROW.
2. Need to remove the underground LP tank on the North West corner of the property. The contractor was told
this at the beginning of their project.

1024 Main Street This CO ha several problems before a CO can be issued. I have talked to the owner of the house and
to the contractor.
need to put in their Water Retention as shown on plans. They have nothing now.
2. Roof needs to drain to the retention area
3. Water retention overflow needs to drain to the street.
705 Sailfish Drive Final Inspection : On this one they need to groom there water retention area a little better and make
the overflow run to the street. I have met with
the contractor and they are aware of what they need to do.
All three of these have my contact information and said they will call me when they are ready for another inspection.

Here is the final As-Built survey for your review. There is only one tree on the property. I just informed the homeowners
that we will have to plant another tree and they are pretty upset.They don't want an extra tree, that is why they paid
almost$10k in mitigation fees to remove the original trees.This should have been brought up at the beginning or
provided to us prior to construction. If there is any way you can waive this requirement if so, I would be grateful. Also, Is
there a specific list of what types of trees are allowed or can we just pick anything?

I just did your CO inspection for 1024 Main Street.Things looked pretty good, but I will double check the survey once
you submit that to make sure. I do have one issue and I'm not sure if we discussed it a permitting.The City has minimum
tree requirements for all lots of one 4 inch diameter tree for every 2500 square feet of lot area. You already have the
one tree in the back yard so you will need one more for this lot. I noticed that there were a couple of trees near the
property line so you may want to check and see where they are at. If there are none then you will have to plant a 4 inch
tree. It can pretty much be whatever tree you want and wherever you want.

This is a 0 Pre Construction ❑Post Construction Subterranean Termite Service Agreement ("Agreement") between Apex Pest Control,
Inc. ("APEX") and the above-named Customer to treat the above referenced Structure(s) for a period of one (1) year for Eastern
Subterranean Termites (Reticulitermes) and Formosan Subterranean Termites (Coptotermes Formosanus), hereinafter referred to in the
plural as"Subterranean Termites," aerial infestations excluded. In consideration for the payment(s) of the fees and tax set forth below,
APEX will service the Structure(s)in accordance with the terms and conditions of this Agreement. Re-treatments may be necessary during
the period of this Agreement and will be provided at no additional cost to the Customer if a reinfestation of Subterranean Termites occurs.
Customer acknowledges that this Agreement is offered for Retreatment Only, unless the upgrade option is selected below. If Customer
does not elect to upgrade to the Repair and Re-treatment Option, then APEX's only obligation will be to re-treat the Structure(s) if an
infestation of Subterranean Termites occurs.
The initial treatment provided under this Agreement is intended to E Prevent(Pre or Post Construction)or 0 Correct(Post Construction
Only)an infestation of Subterranean Termites.
Customer may elect to upgrade the terms of this Agreement to include a ONE MILLION DOLLAR Repair and Re-treatment Option by

By signing this Agreement, Customer hereby acknowledges that Customer has read and fully understands all terms,
disclaimers, limitations, conditions and exclusions contained on the front and back of this Agreement that affect APEX's
obligations to re-treat the Structure(s) or, if the Repair and Re-treatment Option is selected, to repair and re-treat the
Structure(s), under the terms of this Agreement. Customer specifically understands that APEX and Customer are bound only
by the terms of this Agreement and not by any other representation(s)whether oral,written, or otherwise.
BUYER'S RIGHT TO CANCEL:
If this is a home solicitation sale and Customer does not want the goods or services, Customer may cancel this Agreement by giving
written notice of cancellation to APEX before midnight of the third business day after the day Customer signed this Agreement.
