Consider this an Official Notification of Violation, that on February 23, 2009 the City of Atlantic Beach conducted a
Property Maintenance Code Inspection of the aforementioned property. This inspection was the result of a complaint
by the tenant residing at this address. Based on this inspection the following the following violations were identified.
1. International Maintenance Code as Adopted:
a. 604.3 Electrical hazards, kitchen range does not operate properly, electrical outlet to range not installed
properly and outlet in living room inoperable.
b. 305.1 Sanitary condition, mold throughout several rooms.
c. 505.1 Kitchen sink drain piping leaks.
d. 704 No smoke detectors
e. 304.15 Broken doorjamb on master bedroom
f. 305 Kitchen cabinet door and drawers broken L

In an effort to eliminate any undue hardship, I am granting you up to 30 days from receipt of this notice to correct the
following violations,this can be accomplished by:
1. Have an electrical safety inspection of the unit done by a licensed electrician and provide a copy of the
inspection report to Code Enforcement Officer and repair or replace the kitchen range and repair broken
outlets.
2. Have mold tested by a licensed mold remediation company and provide copy of the report to Code
Enforcement.
3. Repair kitchen sink leak.
4. Install smoke detectors in each bedroom and the hallway.
5. Repair the broken doorjamb on the master bedroom door.
6. Repair broken kitchen cabinet doors and drawers.
7. Properly prepare and paint cracked/peeling trim on outside of house.
