On December 7, 2011 at approximately 6:30 pm,Officer Moore and I opped by 1369 Rose Street. We were met at the
door by the resident,Stuart Hawkins(DOB 6/7/61). Mr. Hawkins as d Officer Moore and I to step inside the residence
because of the cold weather. Once inside the residence, I immed' ely noticed an empty bedroom so 1 asked Mr. i
Hawkins who was still residing at the residence. Bobbie Maddox(DOB 11/21/61)walked out of the master bedroom
and stated that she lived at the residence. Ms. Maddox stated that she was the care taker of Mr. Hawkins. Robert Louis
Thomas(DOB 8/20/52)stated that he also lived at the residence. Hawkins, Maddox,and Thomas all agreed that they
were the only residents of 1369 Rose Street. All three stated that the previous residents of 1369 Rose Street, had /
moved out during the last couple of weeks. IrG/
I observed only one furnished bedroom,two sofas in the family room,and a kitchen table.

Thanks Dewayne, please provide some more of the details we discussed when we spoke such as who you saw there,
what they told you,who was sleeping where. Also I need you to go back by after 1800 if possible and confirm again that
those individuals or others are living there. Please detail the situation you observe on this follow up as you did on the
first occasion. your observations will be used as part of Code Enforcement Action so please make the format thorough
as an incident report would be.
