BACKGROUND: After the tragic death of the project's subcontractor, Mr. Jack Ethridge, the
Occupational Safety and Health Administration (OSHA) performed an investigation. During this
investigation, OSHA noted to staff that the ladder cage on the elevated tank, though allowable
when the tank was originally built, would not meet the current OSHA regulations, and stated that
installation of a cable safety climb on the interior and exterior ladders would suffice.
Staff requested a price from the contractor, and the cost to provide and install the stainless steel
cable safety climb devices is $5800 each for the interior and exterior ladders, totaling $11,600.
Although a galvanized device could be provided for $10001ess, the stainless cable is highly
recommended due to the exposure to salt air and water.
BUDGET: Funds for the change order are available in the Water Fund under account nos. 400-
5502-533-4900 ($9500) and 400-5502-533-3100 ($2100). Funds in the first account are
available because an expected permitting fee was not required, and the second account is for
miscellaneous professional services that have not been required to date.
RECOMMENDATION: Approve Change Order No. 4 for Relocation, Installation and
. , Coating of Elevated Storage Tank at Water Plant 2 in the amount of $11,600.

fhe interior of the riser. The interior of the riser is the only one that h~ w be starless
