All material to be excavated seaward of the coastal construction control line as part of construction authorized under this permit shall remain in and be placed in areas seaward of the coastal construction control line. 6. Prior to completion of construction activities authorized by this permit, the permittee shall plant a mix of a minimum of three native salt-tolerant species to restore any disturbed natural area seaward of the authorized structures, including areas disturbed by fill. Dune restoration plantings shall consist of salt- tolerant species indigenous to the native plant communities existing on or near the site or with other native species approved by the Department. Sod composed of non-native grasses is not authorized into dune vegetation, located approximately 190 feet seaward of the control line. Plantings in other areas of the project site shall not include invasive nuisance plant species,such as listed in the Florida Exotic Pest Plant Council's List of Category I and II Invasive Species and including Asparagus fern (Asparagus densiflorus). 7. All lighting shall be installed and maintained as depicted in the approved lighting schematic.Underwater pool lighting shall be permanently fixed to amber or red LED or equivalent and provide no more than ''V2 foot candle per square foot of pool surface area.There may be a decrease in the wattage of each approved lamp and a decrease in the total number of each approved fixture without submitting a modified lighting plan for review and approval. However, if a fixture or lamp is changed to a different type,manufacturer or catalog number for any reason, or if the location of any fixture is changed it shall be submitted for review and approval by the Department prior to installation. (b)The permittee shall conduct the construction or activity authorized under the permit using extreme care to prevent any adverse impacts to the beach and dune system,marine turtles,their nests and habitat,or adjacent property and structures. (c)The permittee shall allow any duly identified and authorized member of the Department to enter upon the premises associated with the project authorized by the permit for the purpose of ascertaining compliance with the terms of the permit and with the rules of the Department until all construction or activities authorized or required in the permit have been completed and all project performance reports, certifications, or other documents are received by the Department and determined to be consistent with the permit and approved plans. (d)The permittee shall hold and save the State of Florida, the Department, and its officers and employees harmless from any damage, no matter how occasioned and no matter what the amount, to persons or property that might result from the construction or activity authorized under the permit and from any and all claims and judgments resulting from such damage. (e)The permittee shall allow the Department to use all records,notes,monitoring data,and other information relating to construction or any activity under the permit,which are submitted, for any purpose necessary except where such use is otherwise specifically forbidden by law. (0 Construction traffic shall not occur and building materials shall not be stored on vegetated areas seaward of the control line unless specifically authorized by the permit. If the Department determines that this requirement is not being met, positive control measures, such as temporary fencing, designated access roads, adjustment of construction sequence, or other requirements, shall be provided by the permittee at the direction of the Department. Temporary construction fencing shall not be sited within marine turtle nesting habitats. (g)The permittee shall not disturb existing beach and dune topography and vegetation except as expressly authorized in the permit.Before the project is considered complete,any disturbed topography or vegetation shall be restored as prescribed in the permit with suitable fill material or revegetated with appropriate beach and dune vegetation. (h)All fill material placed seaward of the control line shall be sand which is similar to that already existing on the site in both coloration and grain size. All such fill material shall be free of construction debris, rocks, clay, or other foreign matter; shall be obtained from a source landward of the coastal construction control line; and shall be free of coarse gravel or cobbles. (i)If surplus sand fill results from any approved excavation seaward of the control line,such material shall PERMITTEE: Idyll Hour,LLC PERMIT NUMBER: DU-497 PAGE 5 be distributed seaward of the control line on the site,as directed by the Department,unless otherwise specifically authorized by the permit. (j) Any native salt-tolerant vegetation destroyed during construction shall be replaced with plants of the same species or,by authorization of the Department,with other native salt-tolerant vegetation suitable for beach and dune stabilization. Unless otherwise specifically authorized by the Department,all plants installed in beach and coastal areas — whether to replace vegetation displaced, damaged, or destroyed during construction or otherwise — shall be of species indigenous to Florida beaches and dunes, such as sea oats, sea grape, saw palmetto, panic grass, saltmeadow hay cordgrass, seashore saltgrass, and railroad vine, and grown from stock indigenous to the region in which the project is located. (k) All topographic restoration and revegetation work is subject to approval by the Department, and the status of restoration shall be reported as part of the final certification of the actual work performed. (1) If not specifically authorized elsewhere in the permit, no operation, transportation, or storage of equipment or materials is authorized seaward of the dune crest or rigid coastal structure during the marine turtle nesting season. The marine turtle nesting season is May 1 through October 31 in all counties except Brevard, Indian River, St. Lucie, Martin, Palm Beach, and Broward counties where leatherback turtle nesting occurs during the period of March 1 through October 31. (m) If not specifically authorized elsewhere in the permit,no temporary lighting of the construction area is authorized at any time during the marine turtle nesting season and no additional permanent exterior lighting is authorized. (n)All windows and glass doors visible from any point on the beach must be tinted to a transmittance value (light transmission from inside to outside)of 45% or less through the use of tinted glass or window film. (o) The permit has been issued to a specified property owner and is not valid for any other person unless formally transferred.An applicant requesting transfer of the permit shall sign two copies of the permit transfer agreement form,agreeing to comply with all terms and conditions of the permit, and return both copies to the Department. The transfer request shall be provided on the form entitled "Permit Transfer Agreement"— DEP Form 73-103 (Revised 1/04), which is hereby adopted and incorporated by reference. No work shall proceed under the permit until the new owner has received a copy of the transfer agreement approved by the Department. A copy of the transfer agreement shall be displayed on the construction site along with the permit. An expired permit shall not be transferred. (p)The permittee shall immediately inform the Department of any change of mailing address of the permittee and any authorized agent until all requirements of the permit are met. (q)For permits involving major structures or activities,the permittee shall submit to the Department periodic progress reports on a monthly basis beginning at the start of construction and continuing until all work has been completed. If a permit involves either new armoring or major reconstruction of existing armoring, the reports shall be certified by an engineer licensed in the State of Florida.The permittee or engineer,as appropriate,shall certify that as of the date of each report all construction has been performed in compliance with the plans and project description approved as a part of the permit and with all conditions of the permit, or shall specify any deviation from the plans, project description,or conditions of the permit.The report shall also state the percent of completion of the project and each major individual component. The reports shall be provided to the Department using the form entitled "Periodic Progress Report" — DEP Form 73-111 (Revised 6/04), which is hereby adopted and incorporated by reference.Permits for minor structures or activities do not require submittal of periodic reports unless required by special permit condition. (r) For permits involving habitable major structures, all construction on the permitted structure shall stop when the foundation pilings have been installed.At that time the foundation location form shall be submitted to and accepted by the Department prior to proceeding with further vertical construction above the foundation.The form shall be signed by a professional surveyor,licensed pursuant to Chapter 472,F.S.,and shall be based upon such surveys performed in accordance with Chapter 472, F.S., as are necessary to determine the actual configuration and dimensioned relationship of the installed pilings to the control line. The report shall state that all locations specified by the permit have been verified and that other construction and activities authorized by the permit have been performed in compliance with the plans and project description approved as a part of the permit and all conditions of the permit; or shall describe any deviations from the approved plans, project description, or permit conditions, and any work not performed. If none of the permitted work is performed, the permittee shall inform the Department in writing no later than 30 days following expiration of the permit. (t) Authorization for construction of armoring or other rigid coastal structures is based on an engineering review and assessment of the design and anticipated performance and impact of the structure as a complete unit. The general site location is shown on Figure 1. At the time of our exploration,the site was developed,with surface cover consisting of grass.An existing residence was located adjacent to the explored area. The site was relatively level with a slight slope downward to the east. Surface water was not observed near planned structural areas at the time of our exploration. 1.2 Project Description You provided project information via several discussions and an email dated November 28,2016. We were provided with a copy of a Map of Survey for the property,prepared by Boatwright Land Surveyors, Inc., last updated June 27,2016. This plan shows the boundary limits for the property,adjacent structures,and the existing roadway adjacent to the site. We were also provided with a copy of the Pool Plan and Details,prepared by Alexander Grace Consulting, Inc.,last dated September 16,2016.We understand that the pool has plan dimensions of 15 feet by 50 feet and the pool bottom will be at approximately elevation 14.7 NAVD. The pool will be founded upon ten 14- inch diameter auger cast in place(ACIP)piles. If actual structure details vary from these conditions,then the recommendations in this report may need to be re-evaluated.We should be contacted if any of the above project information is incorrect so that we may re-evaluate our recommendations. 2.0 FIELD EXPLORATION We performed a field exploration on December 22,2016.The approximate boring location is indicated on the attached Field Exploration Plan(Figure 2). Our personnel determined the boring location using taped measurements from the existing residence and survey controls adjacent to the site.The boring location on the referenced Field Exploration Plan should be considered accurate only to the degree implied by the method of measurement used. 2.1 SPT Boring We located and performed one Standard Penetration Test(SPT)boring,drilled to a depth of approximately 50 feet below the existing ground surface,in general accordance with the methodology outlined in ASTM D 1586 to explore the subsurface conditions within the area of the proposed pool structure. Split-spoon soil samples recovered during performance of the boring were visually classified in the field and representative portions of the samples were transported to our laboratory for further evaluation. A summary of the field procedures is included in Appendix A. 3.0 VISUAL OBSERVATION A geotechnical engineer classified representative soil samples obtained during our field exploration using the Unified Soil Classification System(USCS) in general accordance with ASTM D 2488. EGrou3 of Companies Morris Pool 4.0 GENERAL SUBSURFACE CONDITIONS 4.1 General Soil Profile The detailed boring record is included in Appendix A. It should be understood that the soil conditions will vary away from the boring location.Below the surficial topsoil,the boring generally encountered very loose to very dense fine sand (SP) and loose to medium dense fine sand with silt (SP-SM) to the termination depth of 50 feet below existing ground surface. 4.2 Groundwater Level Groundwater was encountered at the boring location and recorded at the time of drilling at a depth of greater than 10 feet below the existing ground surface.We note that groundwater levels will fluctuate due to seasonal climatic variations,surface water runoff patterns,construction operations,and other interrelated factors. The groundwater depth at the boring location is noted on the Log of Boring record. 4.3 Normal Seasonal High Groundwater Level The normal seasonal high groundwater level is affected by a number of factors.The drainage characteristics of the soils,land surface elevation,relief points such as drainage ditches,lakes,rivers,swamp areas,etc., and distance to relief points are some of the more important factors influencing the seasonal high groundwater level. Based on our interpretation of the site conditions,including the boring logs and the Web Soil Survey which encompasses all of Duval County,we estimate the normal seasonal high groundwater level at the site at the pool area to be approximately 7 to 8 feet below the existing ground surface.It is possible that groundwater levels may exceed the estimated normal seasonal high groundwater level as a result of significant or prolonged rains. 5.0 DESIGN RECOMMENDATIONS 5.1 General Our geotechnical engineering evaluation of the site and subsurface conditions at the property,with respect to the planned construction and our recommendations for site preparation and foundation support,are based on(1)our site observations,(2)the field data obtained,(3)our understanding of the project information and structural conditions as presented in this report, and (4) our experience with similar soil and loading conditions. If the stated structural or grading conditions are incorrect, or should the location of the pool structure be changed,please contact us so that we can review our recommendations.Also,the discovery of any site or subsurface conditions during construction that deviate from the data obtained during this geotechnical exploration should also be reported to us for our evaluation. The recommendations in the subsequent sections of this report present design and construction techniques that are appropriate for the planned construction.We recommend that Ellis&Associates,Inc.be provided the opportunity to review the final foundation plans and earthwork specifications to verify that our recommendations have been properly interpreted and implemented. 5.2 ACIP Pile Foundation Recommendations Due to the proximity of the structure to the Atlantic Ocean,we understand that 14-inch diameter auger cast in place (ACIP) piles will be used to support the pool structure. Based on our experience with similar loading and subsurface conditions,it is our opinion that an ACIP pile foundation system is applicable for this project. 35-24768 2 January 9,2016 EA EIIis&Associatesi1te. rsGroup of Comparies Morris Pool 5.2.1 Allowable Compressive Capacities It is our opinion that ACIP piles,properly constructed,can support the proposed structure when embedded into the soil strata encountered at the site. We have estimated allowable compressive and uplift capacities for 14 inch diameter ACIP piles bearing at a depth of 20 feet below existing grade to be as shown in the following table: ACIP Pile Design Capacity at 20 Feet Below Existing Grade Pile Diameter(inch) Allowable Compressive Capacity Allowable Uplift Capacity(Tons) (Tons) 14 25 8 The above capacities include a theoretical factor of safety 2 for skin friction and 3 for end bearing failure. Our evaluation also included a scourable soil zone of 8 feet below existing site grade. The allowable pile capacity is a function of the structural strength of the grout and the strength of the supporting soil. We recommend the ACIP piles be constructed with grout having a minimum 28-day compressive strength of 4,000 psi.The design pile compressive capacity should not exceed the allowable capacity of the structural member as governed by appropriate codes. 5.2.2 Pile Group Effects We recommend a center-to-center minimum pile spacing of 2.5 to 3 times pile diameter. Using this minimum spacing,we anticipate that any capacity reductions due to group effects of individual piles which are installed within a group of piles should be small and therefore should be considered insignificant in the design of the foundation system. 5.2.3 Settlement With the deep foundation system properly installed to bear at the minimum depths and spacings noted above,at the design load,we estimate that the settlement of an individual pile will be on the order of 0.5 inches or less.This settlement estimate is based upon the use of(1)the field test data obtained during our geotechnical exploration, which has been correlated to geotechnical strength and compressibility characteristics of the subsurface soils beneath the site,and(2)published theoretical and empirical methods of settlement analysis for deep foundations bearing on soils similar to those at this site. 6.0 CONSTRUCTION RECOMMENDATIONS 6.1 Clearing and Stripping Site preparation should consist of clearing the sparse vegetation and any near surface organic topsoil.The clearing/stripping operations should extend within and to a distance of at least five feet beyond the perimeter of the pool construction limits. During grubbing operations,roots with a diameter greater than 0.5-inch, stumps,or small roots in a concentrated state,should be grubbed and completely removed.The actual depths of unsuitable soils and materials should be determined by Ellis&Associates,Inc.using visual observation and judgment during earthwork operations. 6.2 Temporary Groundwater Control Because of the need for densification of the soils at the bottom of the pool excavation and to maintain a dry work area,temporary groundwater control measures may be required if the groundwater level is within 2 feet below the bottom of the pool excavation at the time of construction. Group of Compa.'ies Morris Pool measures become necessary,dewatering methods should be determined by the contractor.We recommend the groundwater control measures,if necessary,remain in place until construction of the pool foundations and grade beams has been completed. The site should be graded to direct surface water runoff from the construction area. Note that discharge of produced groundwater to surface waters of the state from dewatering operations or other site activities is regulated and requires a permit from the State of Florida Department of Environmental Protection(FDEP).This permit is termed a Generic Permit for the Discharge of Produced Groundwater From Any Non-Contaminated Site Activity. If discharge of produced groundwater is anticipated, we recommend sampling and testing of the groundwater early in the site design phase to prevent project delays during construction. Ellis&Associates,Inc.can provide the sampling,testing,and professional consulting required to evaluate compliance with the regulations. 6.3 Excavations The excavation required for the pool construction will be required to meet OSHA requirements. Either a braced sheet pile structure or an excavation having temporary slopes cut back at 1.5 horizontal to 1 vertical (1.5H:1 V)may be used depending on the specific project requirements.The 1.5H:1 V slopes are contingent upon the dewatering system adequately controlling slope groundwater seepage. Sheet piling,if required, should be designed in accordance with OSHA requirements by a Florida Registered Professional Engineer. 6.4 Compaction After completing the clearing and stripping operations and installing the temporary groundwater control measures (if required),the exposed surface of the pool excavation should be compacted with a vibratory drum roller having a minimum static, at-drum weight, on the order of 4 to 6 tons. Typically,the material should exhibit moisture contents within±2 percentage points of the modified Proctor optimum moisture content(ASTM D 1557)during the compaction operations.Compaction should continue until densities of at least 95 percent of the modified Proctor maximum dry density(ASTM D 1557) have been achieved within the upper two feet of the compacted natural soils at the site. Should the bearing level soils experience pumping and soil strength loss during the compaction operations, compaction work should be immediately terminated, and (1) the disturbed soils should be removed and backfilled with compacted structural fill,or(2)the excess moisture content within the disturbed soils should be allowed to dissipate before recompacting. Care should be exercised to avoid damaging any nearby structures while the compaction operation is underway. Prior to commencing compaction,occupants of adjacent structures should be notified,and the existing conditions of the structures should be documented with photographs and survey (if deemed necessary).Compaction should cease if deemed detrimental to adjacent structures,and Ellis&Associates, Inc. should be contacted immediately. We recommend the vibratory roller remain a minimum of 50 feet from existing structures.Within this zone,use of a track-mounted bulldozer,or a vibratory roller operating in the static mode, is recommended. 6.5 Structural Backfill and Fill Soils Structural backfill or fill required for the project should be placed in loose lifts not exceeding 12 inches in thickness when compacted by the use of the above described vibratory drum roller.The lift thickness should be reduced to 8 inches if the roller operates in the static mode or if track-mounted compaction equipment is used. If hand-held compaction equipment is used,the lift thickness should be further reduced to 6 inches. Structural fill is defined as a non-plastic, inorganic, granular soil having less than 10 percent material passing the No.200 mesh sieve and containing less than 4 percent organic material.The fine sand and fine 35-24768 4 January 9,2016 & Ellis&Associates inc. EsGroup of Companies Morris Pool sand with silt, without roots, as encountered in the boring, are suitable fill materials and, with proper moisture control,should densify using conventional compaction methods. Soils with more than 10 to 12 percent passing the No. 200 sieve will be more difficult to compact, due to their nature to retain soil moisture, and may require drying. Typically, fill material should exhibit moisture contents within ±2 percentage points of the modified Proctor optimum moisture content (ASTM D 1557) during the compaction operations. Compaction should continue until densities of at least 95 percent of the modified Proctor maximum dry density (ASTM D 1557) have been achieved within each lift of the compacted structural fill. 6.6 ACIP Pile Construction Recommendations 6.1.1 Construction Techniques ACIP piles should be formed by rotating a continuous,hollow-flight auger to the desired pile tip elevation followed by slow withdrawal of the auger while pumping a mortar grout under pressure through the auger. The pressure of the pumped mortar grout at the auger tip or injection point should be sufficient to(1)fill the pile shaft created by the augering process and withdrawal,(2)prevent"necking"or shaft area reductions due to lateral inward squeezing of any adjacent soft soils,and(3)cause an outward flow of mortar into the adjacent soils.A pressure head within the hollow auger stem equivalent to approximately 10 feet of grout above the adjacent ground surface should be maintained to help verify that a proper grout pressure exists at the injection point.A sudden drop in the sustained pressure head often indicates that a soft zone or void has been encountered and therefore continued mortar injection at this level should be performed until the pressure head has been re-established. Pre-augering and withdrawal of the auger before concreting may result in a reduction of the in-place shear strength characteristics of the adjacent soils and this may require additional pile embedment upon re-augering and concreting.We recommend a reputable contractor,with at least 5 years of experience, install the ACIP piles. 6.6.2 Installation Sequences Construction of ACIP piles located within six pile diameters,center-to-center,should not be performed until the adjacent pile has achieved its initial set, which typically occurs approximately 24 hours after pile construction.This time delay allows the"green" cement grout in the adjacent recently constructed pile to harden, and reduces the potential for loss of grout into the adjacent pile during the augering process. 6.6.3 Steel Placement If lateral loads or uplift will be exerted on the piles,steel reinforcement will be required within the piles. Information contained on the Pool Plan and Details drawing indicates that steel reinforcement has been incorporated into the ACIP pile design. After augering to the desired pile tip level and the auger removed, the steel reinforcement can be statically pushed into recently concreted piles (i.e.,while the grout is still "green"). Typically, up to approximately 10 to 20 feet of steel rebar can be placed by this method. The alignment and concrete cover of the rebar, when placed by this method, can vary. Accordingly, it is recommended that centralizers be placed every 5 feet to ensure proper concrete coverage. 7.0 QUALITY CONTROL TESTING Ellis&Associates,Inc.should be retained to perform the construction materials testing and observations required for this project,to verify that our recommendations have been satisfied. We are the most qualified to address problems that may arise during construction, since we are familiar with the intent of our engineering design. We recommend that our firm be provided the opportunity to make a general review of the foundation plans and the foundation construction and earthwork specifications. If necessary, we will suggest any 35-24768 5 January 9,2016 EA Ellis&Associates enc. rsGroup of Companies Morris Pool modifications that may be required in order to verify that our recommendations have been properly interpreted and implemented. 7.1 ACIP Piles The quality of the pile foundation is dependent upon the skill, experience, and techniques used by the foundation contractor. Since the piles are not visible or accessible for direct inspection after construction, and since problems during installation are not as evident and easily observed as with driven piles, we recommend that a geotechnical engineer,or his/her representative,observe and monitor the auger-grouted concrete pile installations. His/her duties should consist of,but not be limited to, the following: 1. Verify that the piles are augered to the design tip bearing level. 2. Monitor the auger withdrawal rate and grouting operations to help verify that a sufficient mortar grout pressure head is maintained above the injection point during construction. 3. Confirm that the grout pumping equipment is operating satisfactorily throughout the construction process. 4. Record the volume of mortar grout required to construct the pile. 5. Obtain samples of the fluid grout mixture for slump testing and for molding of test cylinders for strength verification of the hardened grout. 6. Monitor the installation of steel reinforcement to verify that the size, length, configuration, and placement of the steel are in compliance with the job specifications. 7.2 Compacted Fill/Backfill A representative number of field in-place density tests should be made in the upper 2 feet of compacted natural soils and in each lift of compacted backfill and fill. Density tests are recommended to verify that satisfactory compaction operations have been performed.We recommend density testing be performed at one location for every 1,000 square feet of pool area. 8.0 REPORT LIMITATIONS Our geotechnical exploration has been performed, our findings obtained, and our recommendations prepared,in accordance with generally accepted geotechnical engineering principles and practices.Ellis& Associates, Inc. is not responsible for any independent conclusions, interpretation, opinions, or recommendations made by others based on the data contained in this report. Our scope of services was intended to evaluate the soil conditions within the zone of soil influenced by the foundation system. Our scope of services does not address geologic conditions,such as sinkholes or soil conditions existing below the depth of the soil borings. The borings were advanced by rotary (or"wash-n-chop") drilling techniques. At 2 '/2 to 5 foot intervals, a split-barrel sampler inserted to the borehole bottom and driven 18 inches into the soil using a 140 pound hammer falling on the average 30 inches per hammer blow. The number of hammer blows for the final 12 inches of penetration is termed the"penetration resistance, blow count, or N-value". This value is an index to several in-place geotechnical properties of the material tested, such as relative density and Young's Modulus. After driving the sampler 18 inches (or less if in hard rock-like material), the sampler was retrieved from the borehole and representative samples of the material within the split-barrel were containerized and sealed. After completing the drilling operations, the samples for each boring were transported to our laboratory where they were examined by our engineer in order to verify the driller's field classification.
