Kosoy, P.E. ~~ Director of Public Works DATE: February 4, 2003 BACKGROUND: Work on the Core City Improvements Project on various streets required that 14 "time and material" items be added to the contract, 7 of which were adjustments to original contract quantities. The items were required due to existing utility conflicts and incomplete information on the design plans. These items are described in the Change Order Item Summary Sheets. The proposed Change Order work for these items added 32 days to the construction contract. There were three "time" only issues due to utility conflicts on Seminole Road, East Coast Drive and City holidays. These items added 18 days to the construction project. The total for Change Order #6, as summarized in the attached documents, is $125,157.35, with a total of 50 days added to the contract. We will have expended 48% of the contingency with this Change Order, however only 47% of the contingency has been expended without the decorative crosswalks which will be covered by other funding at a later date. This percentage tracks well with the money spent through monthly payments ending in January 31, 2003, which are 47% of the total project funds. BUDGET: The original contingency amount, as approved by the Commission is $627,623.09. The six change orders will reduce the contingency by $302,299.36 to $325,323.73. Two structures were added in order to deflect the drainage line away from these trees (S-1200, S-1203). Additionally one yard drain and connecting pipe were added at 5-1215. S-1216 is eliminated due to actual field connections. Total costs for these changes reflect added and subtracted work. Total Cost: $7,289.26 Days Added: 1 2. Additional 2° Services at Plaza: At the request of the COAB, the Contractor added 1- 2" irrigational pressure vacuum breaker and associated hardware for three of the medians on Plaza. Total Cost: $6,510.83 Days Added: 1 3. Water Services on East Coast: The Contract Plans did not indicate the replacement of any water services to coincide with the new water line installation on East Coast Drive from Plaza to Ahern. Direct costs shown are extended from unit- prices established in the bid documents. Total Cost: $19, 584.00 Days Added: 6 4. Inlet Adjustments on 10th Street: In order to eliminate conflicts between the proposed storm drainage and existing sanitary sewer service laterals on 10th Street, the drainage line was lowered approximately 6". As a result, three "V" inlet structures needed to be field modified in order to accommodate the revised drainage line. Costs include labor, equipment and materials for partial demolition and reconstruction of the three structures. Total Cost: $7,117.95 Days Added: 2 5. Modify Service on 12th Street: In order to eliminate a conflict with the root system of a large oak tree at 377 12th Street, Peters Contracting, Inc., an approved subcontractor to PCI placed an additional 100 If of 1 ~2 water service. The price shown includes 10% markup by PCI. Total Cost: $1,430.00 Days Added: 0 6. Sewer Services on 10th Street: Due to conflicts with proposed drainage, 3 private sanitary services were replaced. Curb: Due to previous damage and settlement, 126 If of curb was replaced on Plaza in order to facilitate proper roadway reconstruction and drainage. Additional Driveway Replacement: Additional driveway pavement was replaced on 10th Street due to the need to harmonize existing driveways to revised roadway grades. Total Cost: $500.00 Days Added: 1 12. 15° RCP: Additional drainage pipe required for various modifications on 7th and 10th Street. Total Cost: $720.00 Days Added: 0 13. 4' X 6' Conflict Manholes on 7th Street: Conflict manholes were added at various locations along 7th Street where proposed drainage conflicted with existing sanitary sewer laterals. Adjusting these services would have caused significant impacts to private property. Direct costs shown are extended from unit prices established in the bid documents. Stamped Concrete Sidewalks (8"): The City of Atlantic Beach proposed to construct stamped concrete crosswalks at the intersection of 3rd Street and East Coast Drive, 7th Street and East Coast Drive, and on Sherry Drive south of 3rd Street. The lump sum cost includes the replacement or addition of approximately 37 sy of concrete sidewalk at the crosswalk terminus. Total Cost: $7,125.00 Days Added: 6 TIME AND MATERIALS ISSUES 15. S-902 and S-903: These proposed structures were enlarged significantly in order to accommodate conflicting sanitary sewer lines. Costs are based on actual labor, equipment and materials needed to modify these existing structures. Total Cost: $10,627.48 Days Added: 2 16. Relay Pipe on Plaza: Approximately 50 If of the proposed drainage line was placed at the plan grades when it was determined that significant conflicts with the existing sanitary services could be avoiding by lowering the drainage line. The line was subsequently removed and lowered. The costs represent the actual labor and equipment involved with placing and removing this 50 If of pipe. Total Cost: $4,851.45 Days Added: 1 17. S-P03: This proposed structure was modified in the field due to the existing Bellsouth cables °~~ and utility vaults. Costs are based on actual labor, equipment and materials needed to modify this structure. Total Cost: $3,538.71 Days Added: 1 L8. Additional storm pipe and tie-ins were also included in order to property accept stormwater from the condominium parking lot. Costs are based on actual labor, equipment and materials needed to modify this structure. Total Cost: $11,590.15 Days Added: 2 TIME ISSUES 19. Lost Production on Seminole Road: Due to the existence of several significant underground utilities, numerous drainage modifications were made in order to instal4 the drainage trunk line.on Seminole Road. Additional pipe and structures were added due to alignment changes and additional drainage structures (See Change Orders 3 and 4). During the installation of this trunk tine, the Contractor frequently needed to hand dig excavations and support unidentified cables in order to install drainage. fn many cases, the Contractor's crews were unproductive, spending time securing utilities in the pipe trench or waiting for private utilities to do sa. The additional contract time is based on field records and daily reports documenting inactive or unproductive periods. Days Added: 14 20. Facilities in subgrade on East Coast Drive: Unidentified Bellsouth utilities services and cables were uncovered while constructing the new water main and reconstructing the roadway between 10th Street and Plaza. Replacement of these services caused delays to the Contractor's operations. Days Added: 2 21. Granted Time for 11/29 and 12/24: The City of Atlantic Beach determined that these days should be non-work days due to their proximity to the Thanksgiving and Christmas holiday.
