THIS AGREEMENT WILL BECOME EFFECTIVE UPON PAYMENT IN FULL AND IS SUBJECT TO THE GENERAL TERMS AND
CONDITIONS ATTACHED HERETO. IN SUMMARY,THIS AGREEMENT PROVIDES FOR RE-TREATMENT AND REPAIRS FOR NEW
DAMAGE TO THE SPECIFIC AREAS WITH A LIVE INFESTATION TO AN AGGREGATE OF$250,000 PER BUILDING,SUBJECT TO THE
GENERAL TERMS AND CONDITIONS ENCLOSED HEREIN.
COVERAGE:For a period of five years beginning on the treatment date shown above,Turner Pest Control agrees to provide the necessary
treatrnent for the control of subterranean termites for the covered premises(also referred to as a structure)for the sum of$CHARGED TO
BUILDER,payable on completion of the first treatment by cash,check or cred t card accepted by Turner Pest Control,subject to the general
terms and conditions attached to this Agreement. This Agreement covers subterranean termites only.
REPAIR:Subject to the general terms and conditions attached,if subterranean termite damage occurs In or on the covered premises,while
this agreement is in effect,then Turner Pest Control wit be responsible for the reasonable costs of repairs as determined by Turner Pest
Control,subject to the above lir talion. Turner Pest Control Is not responsible for any repair payments unless Turner Pest Control has
been given the opportunity to inspect the covered premises in the area requiring repair prior to the start of any repair.
Following the expiration of the Initial five year warranty period,subject to the annual renewal fees being paid In accordance with the
terms and conditions by the applicable homeowner's association,TURNER PEST CONTROL reserves the right to completely retreat
the covered premises at a rate determined by TURNER PEST CONTROL and agreed to by both parties prior to the work being
performed,and customer entering Into a new service agreement.
INSPECTIONS: Turner Post Control will perform an annual inspection on all buildings covered under this agreement during the initial Five(5)
year period.

RENEWAL:Following the expiration of the fourth(4)year renewal,TURNER PEST CONTROL,reserves the right to completely retreat the
covered premises at a rate determined by TURNER PEST CONTROL and customer entering into a new servicing agreement
TERMINATION: This agreement may be terminated by TURNER PEST CONTROL at any time if customer falls to comply with its obligations
under the agreement and general terms and conditions attached. The customer must pay the annual Inspection fee by the first day of the
expiration month(initial treatment expiration month Is the expiration month each year)of the previous agreement,or the agreement will be
terminated.
TRANSFER OF OWNERSHIP AND STRUCTURAL CHANGES:The customer agrees to notify TURNER PEST CONTROL of any additions
and/or alterations to the treated structure. This may require the customer to pay for additional treatment,service charges,and/or adjustment
to the annual renewal fee. In the event the property is sold,the property owner at the time of each renewal, if a previous renewal was
purchased, shall have the option of extending the warranty annually after the first year for no less than 4 additional years. TURNER PEST
CONTROL reserves the right to charge a fee to assign the agreement to the new owner and to adjust the annual renewal fee upon transfer of
ownership. In the event the new owner fails to request continued coverage,or does not pay the annual renewal fee, the agreement will
terminate automatically as of the date of the change of ownership. The annual renewal fee will not be refunded in whole or part due to
transfer of ownership of the treated structure.
CHEMICAL SENSITIVITY:At the customers written request,TURNER PEST CONTROL will provide information about the chemicals to be
used in mating the covered premises. If customer or any member of his/her household are or may be sensitive to chemicals or chemica
odors,customer should consult with his/hor physician before chemicals are applied in,on,or under the structure.
ARBITRATION:The customer and TURNER PEST CONTROL agree that any controversy or clam between them,arising out of or relating to
the interpretation,performance or breach of any provisions of this agreement,shal:be settled exclusively by arbitration administered by the
American Arbitration Association,under its commercial arbitration rules,and judgement on the award rendered by the arbitrator(s)may be
entered in any court having jurisdiction. In no event shall either party be liable to the other for indirect,special,or consequential damages or
loss of anticipated profits.
The printed form of this agreement contains all of the terms and conditions of the agreement and no other representations or statements will
be binding upon the parties. The headings are for convenience and ease of reference only,and do not necessarily reflect only what is
