1. I am over the age of eighteen years and have personal knowledge of all facts set forth in this
Qualifying Affidavit.
2. I have submitted a letter of interest and other documentation to the City of Atlantic Beach to be
considered for selection to serve on the City Commission for District 1308, to fill Seat No. 2 recently
vacated by John Stinson.

4. I acknowledge and am familiar with the qualification requirements for City Commission members
as set out in Sections 5 and 6 of the Atlantic Beach Charter and state the following:
a. I reside within District 1308, as that District is described in Section 2-22 of the Code of Ordinance
of the City of Atlantic Beach.
b. My principal place of abode has been within the City of Atlantic Beach for at least two (2) years
as of the date hereof, making me a "full time resident" as described in said Section 6 of the
Charter.
c. I am a registered voter in the City of Atlantic Beach.
d. I do not hold any elective office.
e. I have not been convicted of any crime punishable by more than one (1) year of imprisonment
or a crime involving moral turpitude.

I have lived in Atlantic Beach for 34 years and I have never been arrested or charged with any
crime. I am applying for the vacant city commission .seat, vacated by Mr. Stenson. I believe that
average people in the average community can change the world. It can be done with common
sense, determination, persistence and patience.

OBJECTIVE:
I believe that average people in the average community can change the world. It can be done
with common sense, determination,persistence and patience. I know that I am one of these
people and I seek an opportunity to do so in my Atlantic Beach community.
EDUCATION:
St Vincent's Hospital School of Nursing New York,NY; Registered Nurse
WORK EXPERIENCE:
2015-present Beaches Emergency Assistance Ministry, Director of Food Services
Manage two food pantries, and soon, a community food bank. Supervisor
for 50 volunteers,work closely with Feeding Northeast Florida and Farm
Share. Coordinate resources with community partners; Publix, Winn
Dixie, Target,Native Sun, Walmart. Redistribute to agencies in need;
Pablo Towers, Pablo Hamlet, Rising Tide, Mission House, and Meals on
Wheels. Work closely with registered dietician to provide the most
nutritious food to our most needy clients.
2013-present Beaches Emergency Assistance Ministry, Garden Manager
Planned, obtained funding from a corporate partner(Home Depot),
worked on design, irrigation, building and planting a small farm of 45
raised beds 4'x12'. The Grace Garden produces 10,000 pounds of fresh
organically grown, vegetables and fruits for our two food pantries. I work
with a core of regular volunteers, UNF students, Master Gardens from UF
Agricultural extension office, church groups, elementary and high school
students, clients and each semester a student from The North Florida
School of Special Education.
2011- 2013 Atlantic Beach Elementary School, Garden Coordinator
Planned, funded, (FuelUp to Play 60 and PTA)built, irrigated and planted
with the children. The Children's Garden by the Sea is a beautiful garden
of 22 raised beds 4'x8', one for each class K-5. We have "Veggie Voting"
where each class chooses what to plant and"Lettuce Lunches", Broccoli
Cheese soup and steamed green beans, grown and harvested by the kids at
Movie Night, Science Night and Reading Night. We are in the process of
gaining approval to serve the garden food in the cafeterias.
1990-2011 ELA Medical (later The Sorin Group), Rep principle
High level medical sales; pacemaker and defibrillators. Had 4 employees.
Covered Jacksonville, Gainesville and Ocala.
1970— 1990 FJC north campus, Clinical nursing,part time instructor
Worked, managed and taught Intensive Care, cardiology and
cardiovascular nursing.
ICU, CCU Baptist Medical Center, Senior Team Leader
Supervised the nursing care, scheduled staff; and trained new nurses in a
24 bed acute care unit. Coordinated bed utilization with the medical staff.
Riverside Hospital Jacksonville, CV Nurse Clinician
Created a cardiac rehabilitation program for post MI and CV surgery
patients;taught EKG interpretation to acute care nurses and monitor
technicians; supervised nursing care of patients with cardiovascular
diseases.
Duval Country Medical Center (now UF Health), Staff Nurse
Provided bedside nursing care to patients in the newly created
ICU/CCU unit.
STRENGTHS:
I am passionate, energetic, flexible and hard working. I embrace partners and people from all
walks of life. All this helps me facilitate the achievement of common goals among disparate
groups. I care about the people in my community and I would be a rational, effective and
empathetic city commissioner.
