NOW THEREFORE, I, the undersigned circuit judge, declare THOMAS J.AYCOCK,III and
S.GARDNER RIEL duly qualified under the laws of the State of Florida to act as limited co-guardians
of the person and property of Cherye D. Aycock,with full power to exercise the following powers
and duties pertaining to the Ward's person and property:
1. to determine the Ward's residence and caregivers(although the Ward will not be
forced against her will to return to live at the Empire Point house.);
2. to review accountings from any Trustee holding assets for the benefit of the Ward;
3. to investigate any transactions relating to the accountings of those assets,to object to
those accountings, and if necessary, to sue with respect to those accountings;and to
share trust accountings with family members if the limited co-guardians believe it to
be in the Ward's best interest;
4. to contract or enter into any transactions on behalf of the Ward when by its terms,the
contract or transaction involves matters aggregating more than$1,000 and to sue and
defend lawsuits relating to the same; and
5. to make health care decisions, however, the limited co-guardians are required to
consult with the Ward to the extent she is able to participate in those decisions. The
limited co-guardians may share general information regarding the Ward's medical
treatment with other family members to the extent the Ward's confidentiality and
privacy rights are not violated.
The Ward has the right to participate in all decisions made by the limited co-guardians,and
the limited co-guardians will give the Ward's wishes significant weight as long as her wishes are
reasonable and in her best interest.
