BACKGROUND:, Work on the Core City Improvements Project on Seminole Road
and 11`h Street uncovered some unforeseen, unorthodox water
main configurations.
As-built plans did not show, and the design engineer nor the
Atlantic Beach Utility Department was aware, that two parallel
water mains crossed 1.1 cn Street, a 1.0" and a 12" diameter line.
Since the 10" diameter line is the main running down the west side
of Seminole Road, it needed to be maintained. The 12" diameter
line was crossing to the 12" main to be abandoned, so this line
needed to be abandoned. For future maintenance, it was necessary
to remove unnecessary piping, install a new 12x12 cross at the
southwest corner of the intersection and install new valves.
Sketches of the area before and after the construction are attached.
We could not stop the project and move elsewhere, since this area
of Seminole Road is on the critical path of the project and any
delay would have been more costly than the actual work to be
performed., The contractor was directed to perform the work on a
time and materials basis, which was tracked very carefully by GAI
Consultants. The contractor submitted the costs and, after
negotiation with GAI, Change Order 4 was submitted for approval.
BUDGET: The original contingency amount, as approved by the Commission
. is $627,623.09. The four change orders will reduce the
. contingency by. $95,059.36 to $532,583.73.

In compliance with the General Conditions of the above the referenced contract, PETTICOAT
CONTRACTING, INC. and THE CITY OF ATLANTIC BEACH do both hereby agree that PETTICOAT
CONTRACTING, INC. shall make the following changes, additions or deletions to the work specified
in the specifications and plans.
Modify existing watermain configuration at the intersection of Seminole Rd. and 11th
Street to provide far enhanced operation and maintenance. Exhibit 1 & 2 detail the
proposed modifications. 4 days of contract time will be added due to this extra work.
The cost of the changes under this Change Order will increase the initial Contract in the
amount of: $ 27,445.37
