APPLICATION FORM ADDENDUMS
I. ADDENDUM #1: NEEDS STATEMENT
Coastal Kicks Soccer, Inc., is primarily a soccer training and development organization for children aged
three to twelve REGARDLESS OF economic status, gender, cultural/racial/ethnic backgrounds, and
levels of play. The CEO and Executive Director of Coastal Kicks Soccer, have together nearly fifty years
of experience in youth soccer across 10 states and nationally. Based on that experience plus ten years of
CoAB residency, we have identified a significant need throughout Atlantic Beach. There is a huge gap
in access to reasonable, professional soccer training for the children of Atlantic Beach, regardless of
economic capabilities. Coastal Kicks was established as a non-profit specifically to provide our services
to particularly underserved and economically disadvantaged populations.
This need has been confirmed by the growth of our local programs and the ongoing communication we
maintain with our neighbors, community organizations, and players' families. The only option for AB
residents interested in professional -level soccer training is the very large, extremely expensive soccer
clubs, that employ a "competition model", as opposed to our "development model". There are some rec-
level soccer opportunities near AB, but they do not provide the same level of training by soccer
professionals.
II. ADDENDUM #2: PROGRAM DESCRIPTION
A. Program Goals and Objectives
1. To provide top-rated, as measured by player family surveys, weekly year-round soccer
training and development to approximately 210 Atlantic Beach children.
2. To provide player -centered weekly games for 60-80 Atlantic Beach children.
3. To provide a week-long summer camp opportunity for approximately 30 Atlantic Beach
children.
4. To provide access to our programs to economically disadvantaged families, through our
Scholarship Fund totaling $825.
5. To provide parent education on the current standards for US Soccer and how to best develop
their player, through special appearances at events and providing two Family Fun Day and
Free Clinics open to all AB residents free of charge.
6. To provide support for CoAB recreational events and programs, through personal
participation, social media and website support, identifying volunteers, or whatever is needed.
7. To expand our program to be more popular for older kids by the formation of teams that
would receive additional training and experience a gradual entry into travel soccer through
league play and festivals/tournaments.
8. To expand our program by offering 5 -a -side soccer called "Futsal", the only FIFA -sanctioned
small -sided version of soccer.
Benefits for the Community
1. More Atlantic Beach children will be able to experience a player -centered soccer experience
that will be both enjoyable and productive with respect to skill development.
2. More economically disadvantaged children will have their first opportunity to play soccer.

3. Providing recreational opportunities to all AB children across the spectrum will enhance
individual physical and mental health, group dynamics, respect for others, sense of
community, respect for education, and their love of the game.
4. More AB parents will have a clearer view of the sometimes -complicated world of organized
youth soccer.
B. Program Activities
Coastal Kicks programs are fully insured with Liability Certificates naming CoAB as an
additional insured. In addition, Coastal Kicks will agree to any Indemnification clause required.
All grant -funded items requested are directly related to the program's activities. No grant funds
will be used for capital expenses or to offset other funding sources.
There are four carefully -interwoven segments of the Coastal Kicks Soccer development model —
1. "Minnows": our youngest players, three to four year olds in groups of 8-12, meet once
per week for 45 minutes for 9 -week seasons in Fall, Winter, and Spring, focus is on
building enjoyment with the ball at their feet, and playing games that develop individual
ball skills like dribbling and striking (kicking).
2. "Dolphins" and "Marlins": five to seven year olds, in groups of 10-14, train once per
week for 60 minutes for 10 -week seasons in Fall, Winter, and Spring, focus is on
individual player development of fundamental technical skills, PLUS one game per week
for those seasons, where a kid -centered environment allows for developing a passion for
the sport and developing critical decision-making and problem -solving skills.
3. "Barracudas" and "Sharks": eight to twelve year olds, in groups of 10-16 train once per
week for 60 minutes for 10 -week seasons in Fall, Winter, and Spring, focus is on
individual player mastery of fundamental technical skills and development of
fundamental tactical awareness and advanced technical skills, PLUS one game per week
for those seasons, where a kid -centered environment fosters refining individual skills and
group/team tactical thinking and developing critical communication skills.
4. Teams: for 8-12 year olds, players are offered the opportunity to play on a team of
Coastal Kicks players that play 7v7 (8s -9s) and 9v9 (10s -12s) in league play, "friendlies"
(scrimmages), and 2-3 festivals or tournaments per year, and train together as a team
twice per week.
Program History
o Combined, the two principals of Coastal Kicks have nearly 50 years of experience in all
aspects of soccer across all age groups from three to adult, gender, level of play, and
cultural/ethnic backgrounds. Michael Stauch and Jay Golomb also are both educators, of
players, parents, and other coaches, and have developed and refined a development -focused
model that has been successfully implemented for the last five years in Jacksonville.
o Coastal Kicks unconditionally guarantees a unique soccer development opportunity to the
AB community that fosters enjoyment of the game and obvious development of individual
and team skills. It is an environment where kids can feel comfortable to develop at their own
rate and is VERY different from other more expensive and less community -focused, or less
professional, alternatives.
o Coastal Kicks is very interested in extending our experience with Futsal, the only FIFA -
sanctioned small -sided soccer, to the Atlantic Beach community, bringing a totally new
method of training and play to the area. We can begin Futsal as soon as we can identify a
potential gymnasium.
o Coastal Kicks Soccer, Inc., provides family-oriented and community -focused programming
to all, regardless of background or ability to pay.
o Coastal Kicks is a non-governmental non-profit completely free from religious activities.

C. Advertising
Coastal Kicks employs a variety of advertising strategies to publicize our programs, including a
well-designed fully -functional website and SportsEngine platform for easy registration, payment,
and program tracking, an active presence on social media platforms, use of community-based
blogs and websites, as well as signage and print media (flyers, brochures, posters). Our Marketing
Plan also includes public appearances, supporting local community or government events, and
providing Family Fun Day and Free Clinics open to all AB residents.
III. ADDENDUM #3: ACCESS
Coastal Kicks is implementing an Access Plan for as many of our sites as possible, to include --
a. providing a way to facilitating access to our programs for children who do not live in the
immediate neighborhood or community or who lack transportation options;
b. partnering with CoAB and CoJ to provide full -access transportation vehicles;
c. identifying more underserved community sites to take our programs to; and
d. identifying alternate funding sources to fund a transportation component.
IV. ADDENDUM #4: EVIDENCE -BASED MISSION
There is a grassroots movement in youth soccer to reduce/eliminate the "pay to play" system currently in
place, which Coastal Kicks is firmly committed to. The movement to replace the current competition -
based model with one focusing exclusively on individual player development prior to age 12 is well-
grounded in evidence -based education/learning research publications, well-documented international
experience, and the principles of FIFA (the international governing body for soccer).
Coastal Kicks overriding Mission is based on this widely -accepted development model, combined with
our own demonstrated history of success. Based on the research in the field and our prior experience and
success in Atlantic Beach and other areas, we can reasonably expect to provide effective soccer training
and development to Atlantic Beach children.
V. ADDENDUM #5: AFFORDABLE FEES
To ensure the participation of economically disadvantaged children, minimal fees are charged to cover all
operating expenses, and all "profits" are converted to a separately maintained Scholarship Fund. This
fund is also supported by an optional $25 donation for all registrants while they are registering (to enable
those less fortunate to participate), sponsor -funded scholarships, and assorted fundraising activities.
We publicize our Scholarship Fund to ensure that we can provide our services regardless of the ability to
pay. We have a simple, unobtrusive process to identify those in need of financial assistance and quickly
agree to fund them through our Scholarship Fund.
VI. ADDENDUM #6: EVALUATION PLAN
Coastal Kicks relies on open, direct communication with all families to provide constant feedback on how
we're doing. We will conduct a full Evaluation Plan for all grant -funded activities, that will include
1. pre- and post-test parent/player Satisfaction Surveys;
2. program improvement feedback from staff;
3. data collection through the SportsEngine platform that tracks all program metrics and measures
participation, producing useful and actionable data;

4. data collection through Mail Chimp and social media platforms to guide future marketing and
advertising;
5. welcoming site visits before and during program implementation; and
6. completing an Evaluation Report within 30 days of program completion for presentation to the
grant source and disseminated to the public through our website and printed materials.
VII. ADDENDUM #7: RESOURCING
Coastal Kicks employs a diverse Resourcing Plan, as reflected in the attached budget, to leverage
additional resources that includes various collaborations with other organizations. The components of the
Resourcing Plan are:
1. Cash Sponsorships: Sports Benefits and Angel Kids Pediatrics have recently signed Sponsor
Agreements to support our programs for Fall and Winter, and we have a goal to obtain an
additional $1,000 this year from other sponsors.
2. In -Kind Sponsorships: Several sponsors prefer to provide in-kind contributions to support our
programs. Although in-kind contributions do not contribute cash, they are designed to reduce or
eliminate the cost of certain items. For example, a prior sponsor provided give-away items to our
players which we would have had to purchase, and other sponsors cover the cost of tee shirts and
jerseys through their generosity in return for logo exposure.
3. Fundraising: We are starting a GoFundMe $1,000 campaign to raise money for our Scholarship
Fund. Through SportsEngine, Coastal Kicks is part of a network that certified and lists us as a
potential benefactor for corporate -matched donations of their employees.
4. Grants: We are pursuing several grant sources, from private foundations to government grants,
utilizing partnerships with other non-profit or governmental agencies, such as City of
Jacksonville.
