Kosoy, P.E., Director of Public Works ~~~ DATE: November 2, 2001 BACKGROUND: We advised the City Commission at the 10/8/01 meeting that we had encountered unforeseen underground items under Sherry Drive. These items included an abandoned concrete vault at 6'h Street, logs at 5th Street and trees at 4s' Street. We requested the geotechnical engineer, Ellis & Associates, Inc., to suggest various options for dealing with the problem, and we received a letter from them, dated October 9, 2001. On October 10, 2001 a letter was sent to Callaway Contracting, Inc., requesting prices for the various solutions. We received prices from Callaway in a letter dated. October 17, 2001. These prices compared reasonably well with the Florida Department of Transportation's average weighted prices for similar work. An analysis of the costs for each proposed solution is attached, with the total cost for Option 1 being $75, 143.25, Option 2 being $33,808.10, and Option 3 being $25,556.90. A fourth option would be to use Option 1 between 4th and 5th Streets (where a new 29" x 45" stonnwater pipe will be installed), and Option 3 between Sth and 6th Streets, (where no drainage work is planned), for a total of $55,406.10. BUDGET: The total additional work is proposed to cost $75,143.25, which, added to the previously. approved Change Order 1 dated August 13, 2001, will be funded from previously approved 10% contingency amount on the project, which is $126,951.02. This will leave a balance of $31,372.49 within the project contingency. RECOMMENDATION: Approve Qption 1, a full depth removal between 4"h and 6th Streets for $75,143.25, adding 60 calendar days to the current contract completion date 1/26/02, with a new completion date. of 3/27/02. Box 11435 Jacksonville, FL 32239 RE: Change Order Prices for Rebuilding of Portion of Sherry Drive Dear Mr. Kirby: As discussed over the past several weeks, we have encountered a great deal of tree logs, trees, and a brick vault, predominantly between 4`h and 6`" Street. I discussed this matter with our geotechnical engineer, Ellis & Associates, Inc., and am attaching their report dated 10/9/01. Please furnish prices for each of the three possible remedies listed below for rebuilding Sherry Drive in this area: 1) Completely demuck the area of unsuitable material. Provide this cost in cubic yards of unsuitable material to be removed and select backfill to be replaced. Assume the worst case scenario that the entire width of Sherry Drive will need to be demucked to a depth of 6', between 4"' and 6`h Street. 2) Over-excavate to a minimum depth of 3' below the pavement base, and replace with compacted structural backfill. (Speclfication attached.) Provide this cost in cubic yards of over-excavation and compacted structural backfill. 3) Over-excavate to a minimum depth of 2' below the pavement base, install a woven geotextile fabric (Marafi 500X or equivalent), and place compacted structural backfill. Provide this cost in cubic yards of over-excavation and compacted structural backfill, and square yards of geotextile fabric. Please provide these additional costs by Friday, October 19, 2001. Structural fill is defined as anon-plastic, inorganic, granular sail having less than 10 percent material passing the No; 200 mesh sieve end containing less than 4 percent organic rnate:ial. Structural fill orbaekfill required for hacl~lling the aver-excavated areas should bepIaeed in Loose liRs not exceeding 12 inches in thickness and campacud by the use of a vibratory drum roller having e minimum statiq at-drum weight on the order of 4 tons. Typically, thz material should exhibit moisture contents within ±2 percent of the Modified Proctor optimum moisture content {ASTM D 1557) during the compaction operations. Compaction should continue until densifies of at least 95 percent of the Modified Proctor maximum dry. density (ASTM D 1557) have been achieved within each 12-inch iift. Ifhand-held compaction equipmentis used, the lift thickness shouldbereduced to 6 inches. It should be noted that soils with more than lOto 12 perce:tt passing the No.20o sieve will be more difficult to comp$et due to their nature to retaiir soil moisture and tray require drying.
