The house is a split-level wood frame home with stucco finish, built in 1995. w v w 2. The floor is concrete slab on grade. > 7-1 v7 cc w W 3. The foundation is a thickened perimeter footing. cc cc 4. According to the JAXGIS Property Map, the home was built on a marshland. 5. Both the north and south wall of the home are experiencing settlement. 6. It appears that some erosion has occurred and has resulted in voids under the floor slab along the north and south walls. 7. There is a vertical crack beginning to form between the floor slab and the framing due to settlement. (see photos) 8. The interior drywall joints at the corners are beginning to separate due to settlement. (see photos) 9. Measurements along the north and south walls are up to 1.4 inches below the original finished floor. �0/ Alpha Foundation Specialists, Inc. The cause of the settlement is unknown,but most likely due to a combination of inadequate site preparation, erosion, and decomposing organics in the supporting soil. 11. Soil borings were not taken. 12. It is likely that the settlement will continue to occur unless the foundation is stabilized. 13. To stabilize the foundation, we recommend installing 11 three inch helical piers along the south wall, and 7 three inch helical piers along the eastern portion of the north wall for a total of 18 three inch helical piers. These helical piers shall be up to 14 feet long using a standard bracket and spaced a maximum of 7.0 feet. See attached sketch. 14. Pier spacing may exceed 7.0 feet to avoid placement directly under a window or door. 15. Pier spacing may also exceed 7.0 feet to avoid interference with the pool pump. 16. To fill the voids under the concrete floor slab along the north and south wall, we recommend using Po1yLEVEL as shown in the attached sketch. The house is a split-level wood frame home with stucco finish, built in 1995. 2. The floor is concrete slab on grade. 3. The foundation is a thickened perimeter footing. 4. According to the JAXGIS Property Map,the home was built on a marshland. 5. The south wall of the home is experiencing settlement. 6. It appears that some erosion has occurred and has resulted in voids under the floor slab along the south wall. 7. There is 4-vertical crack beginning to form between the floor slab and the framing due to settlement. (see photos) 8. The interior drywall joints at the corners are beginning to separate due to settlement. (see photos) 9. Measurements along the south wall are up to 1.4 inches below the original finished floor. \l' f(67 Alpha Foundation Specialists, Inc. Foundation Stabilization 2341 Fiddlers Lane Page Two 10. The cause of the settlement is unknown,but most likely due to a combination of inadequate site preparation, erosion, and decomposing organics in the supporting soil. 11. Soil borings were not taken. 12. It is likely that the settlement will continue to occur unless the foundation is stabilized. 13. To stabilize the foundation, we recommend installing 11 three inch helical piers along the south wall. These helical piers shall be up to 14 feet long using a standard bracket and spaced a maximum of 7.0 feet. See attached sketch. 14. Pier spacing may exceed 7.0 feet to avoid placement directly under a window or door. 15. To fill the voids under the concrete floor slab along the north and south wall, we recommend using Po1yLEVEL as shown in the attached sketch.
