In response to those comments, attached please find the following items for final plan review and approval: 1. Two copies of the warranty deed for 761 Atlantic, 2. Two copies of the Shoppes of Lakeside merger documents, 3. Two copies of the Sunbiz reports for 412 Boardwalk,Inc.and Shoppes of Lakeside,Inc. 4. Completed and signed building permit application, 5. Two copies of the revised signed and sealed survey, and 6. Four sets of revised civil plans. The following is a summary of your preliminary review comments, and our responses to those comments: Zoning Comment: The submitted survey does not show the recent change to the property line in the northeast corner. The property line was changed so that the walk-in coolers were entirely on the neighboring property. Response: The civil plans have been updated to reflect the revised property boundaries. Comment: Please submit a landscape plan for the proposed changes. Response: As discussed with Brian Broedell on January 136, a landscape plan is not required because the construction cost is well below 25% of the assessed building value. The single palm tree to be relocated is shown in its new location on the Site Geometry Plan. Comment: Please submit an access agreement with 751 Atlantic Blvd as well as an agreement to relocate the palm tree. Response: As discussed with Brian Broedell on January 13t1, both properties are owned by the same person, and therefore an access agreement is not required 763 Atlantic Boulevard is owned by 412 Boardwalk, Inc. as evidenced by the attached warranty deed, and 751 Atlantic Boulevard is owned by Shoppes of Lakeside, Inc., successor by merger with OUR Properties, Inc. as evidenced by the attached merger documents. Chris Hionides is an officer of both corporations as evidenced by the attached Sunbiz reports. Building Comment: Building Permit Application Form is not complete. Please resubmit Application with all boxes checked and all information provided. Response: A completed building permit application is attached. Comment: Permit Application is not signed by licensed Contractor. Response: The GC for the project has signed the attached building permit application. Comment: Please submit two copies of the current original survey, signed, sealed, and dated. Response: The updated survey with revised property boundaries is attached. Comment: Please indicate the required two accessible parking spaces, including access aisles and signage, per FAC,Section 208 and Chapter 5;and curb ramps, per FAC,Section 406. Response: As discussed with Dan Arlington on January 3'1, adequate ADA parking exists at this site for this project, as confirmed by a site inspection, and this comment can be disregarded. Please distribute the attached plans for final review and approval of the proposed access improvements.
