BACKGROUND: ~ The janitorial contract for the cleaning and maintenance of City
buildings was advertised for bid last month. There was only one
(1) bid that was received, and it was for over $52,000. Apparently,
many of the businesses that might have bid on the project did not
want to post the insurance required for qualification. To put the
existing bid into perspective, the City of Atlantic Beach spent over
$32,000 in 1998 on building maintenance.
` In looking at various options, it is feasible for the City to establish
a Building Custodian position with full benefits, and still reduce
the annual cost to the city for building maintenance. Additionally,
it is desirable to have someone with tivhom the other department
members become familiar. This provides for better
communication and improved service, especially when there are
special needs or problems to be addressed. Although the previous
City Manager made a similar attempt in the past to hire someone to
perform this service, the position at that time had no benefits.
A full time position with benefits can be established @ 7.61 per
hour with full benefits, and the annual cost to the City will be
approximately 21,929 per year. The city will also have to provide
cleaning equipment and supplies for this position tivhich can be
anticipated at less than $1,000 for start up costs and cleaning in this
fiscal year.
In the supervisory structure of the City, this position would be
answerable to the Finance Department, consistent with the existing
process for building maintenance.

RECOMMENDATION: We recommend that the City Commission authorize the creation of
a full time Building Maintenance position for the City of Atlantic
Beach.

No cleaners or chemicals are to be used or sprayed around any electronic
equipment. Anything found on floors is to be placed on table or countertop,
not thrown away.
CONTRACTOR will furnish all disinfectants, trash bags, vacuum; brooms, dust
pans, and any other necessary cleaning equipment and supplies.
In addition to other requiremeents outlined in these documents, all employees
of the successful bidder who will be assigned to these City facilities must

bVhat Is Backup Withholding?-Persons
making certain payments to you must
vrithhold and pay to the IRS 31% of such

royalties, nonemployee pay, and certain

Failure To Furnish T1N.-If you fail to
furnish your correct TIN to a requester, you
are subject to a penalty of S50 for each
such failure unless your failure is due to
reasonable cause and not to willful neglect.
Civil Penalty for False Information With
Respect to Withholding.-If you make a
false statement with no reasonable basis
that results in no backup withholding, you
are subject to a SS00 penalty.
Criminal Penalty for Falsifying
Information.- Willfully falsifying
certifications or affirmations may subject
you to criminal penalties including fines
and/or imprisonment.

Specific Instructions
Name.-If you are an individual, you must

How To Get aTIN.-If you do not have a
TIN, apply for one immediately. To apply
for an SSN, get Form SS-5 from your local
Social Security Administration office. Get
Form W-7 to apply for an ITIN or Form
SS-4 to apply for an EIN. You can get
Forms W-7 and SS-4 from the IRS by
calling 1-800-TAX-FORM
(1-800-829-3676).
If you do not have a 71N, vrrite "Applied
For" in the space for the TIN, sign and
date the form, and give It to the requester.
For interest and dividend payments, and
certain payments made with respect to
readily tradable instruments, you will
generally have 60 days to get a TIN and
give it to the requester. Other payments
are subject to backup vthholding.
Note: Writing Applied For" means that
you have already applied for a T!N OR that
you intend to apply !or one soon.
Part 11--For Payees Exempt From
Backup Withholding
Individuals Including sole proprietors) are
not exempt tcom backup withholding.
Corporations are exempt from backup
withholding for certain payments, such as

. Absolute Cleaning supplied temporary custodial services to Putnam
County from October 26, 1998 through January 15, 1999, under the
direction of the Putnam County Public Works Department. The
contracted service areas consisted of a combination of both
• daytime and nighttime cleaning and included such functions as
sweeping/mopping floors, vacuuming carpets, emptying trash,
stocking paper projects; and general office and restroom
cleaning. The paper products for the buildings under contract
were supplied by the county, and we found the contractor to be
both responsible and conscientious with the disbursement of these
supplies. In addition to the general terms of the contract, the
contractor also responded to additional departmental requests and
concerns in a timely and efficient manner.

Purpose of Form.-A person v+H'o is
required to file an information return vrith
the IRS must get your correct taxpayer
identification number (fIN) to report, for
example, income paid to you, real estate
transactions, mortgage interest you paid,
acquisition or abandonment of secured
property, cancellation of debt, or
contributions you made to an IRA.
Use Form W-9 to give your correct TIN
to the person requesting it (the requester)
and, when applicable, to:
1. Certify the TIN you are giving is
correct (or you are waiting for a number to
be issued).
2. Certify you are not subject to backup
withholding, or
3. Claim exemption from backup
withholding if you are an exempt payee.

What Is E3ackup Withholding?-Persons
making certain payments to you must
~withho-d and pay to the IRS 31% of such
,payments under certain conditions. This is
called 'backup withholding." Payments
that may be subject to backup withholding
include interest, dividends, broker and
barter exchange transactions, rents,
royalties, nonemployee pay, and certain
payments from fishing boat operators. Real
estate transactions are not subject to
backup withholding.
It you give the requester your correct
TIN, make the proper certifications, and

Penalties
Failure To Furnish 7IN.-If you fail to
furnish your correct TIN to a requester, you
are subject to a penalty of 350 for each

false statement with no reasonable basis
that results in no backup withholding, you

Criminal Penalty for Falsifying

Specific Instructions
Name.-1f you are an individual, you must
generally enter the name shown on your
social security card. However, ff you have
changed your fast name, for instance, due
to marriage, without informing the Social
Security Administration of the name

Part 1-Taxpayer Identification Number

MAJOR JOB RESPONSIBILITIES:
-Cleans City buildings and facilities;
Sweeps, mops and dusts offices, desks, blinds, file cabinets, window sills, bookshelves, etc.;
Cleans interior glass/mirrors;
Cleans all restrooms, fixtures, and common areas including sanitizing and deodorizing toilets,
lavatories, and floors; checks and stocks supplies; ensures that all lavatories are working.
Spot clean, as needed, door jams, ledges, light switches, etc.;
Gathers and disposes of refuse, litter and trash in designated facilities;
Empties trash cans and lines with plastic liners;
`Washes windows, walls, woodwork, bathroom fixtures;
Operates carpet shampooer; vacuums and cleans carpets;
Loads and unloads cleaning and polishing equipment and materials to be transported by truck;
Prepares quantities of cleaning and polishing materials appropriate for use from bulk containers;
Keeps buildings and areas around buildings free of debris and trash;
Performs routine security tours of the building and grounds to which assigned and reports unusual
observations to superior;
Performs other duties as required.
