Kosoy Director of Public Works~~%¢Z~ DATE: April 15, 1999 BACKGROUND: Staff sent out invitations to bid for milling and paving of the following streets for the Public Works Department: ~eminole Road from Atlantic Blvd. to the S-way intersection Beach Ave. from Ahem to 7`~ Street Beach Ave. from 10~' Street to 14`~ Street 16`'' Street from Ocean Blvd. to Beach Ave. Sandpiper Lane from Plaza to Donner Road Sargo Road from Plaza to Cavalla Road Skate Road from Plaza to Cavalla Road Cutlass Drive from Royal Palm to Aquatic Drive aratoga Circle ~, Forrestal Circle And Dust Control on the following streets: Cypress Street Dora Street Kestner Road ' Carnation Street West Plaza Gladiola Street Stocks Street (900 Block) The bid opening was held on April 15, 1999 at 3:00 p.m. and we received four complete bids. The bidders were as follows: The Milestone Company $176,458.15 J.B. Coxwell Contracting, Inc. $246,448.20 Duval Asphalt Products, Inc. $233,310.00 Atlantic Coast Asphalt Company $223,614.00 This work will be funded from the Roads and Streets Fund in the Capital Improvements Program (pg. 150 of the FY98-99 Budget). REVIEWED BY CITY MANAGER: T CITY OF ATLANTIC BEACH BID NUMBER 9899~20 PURCHASING DEPARTMENT 1200 SEMINOLE ROAD ATLANTIC BEACH, FLORIDA 32233 (904) 247-5818 REQUEST FOR BIDS CONTRACT -MILLING AND PAVING FOR THE PUBLIC WORKS DEPARTMENT PART 1 -GENERAL 1.1 PE: The work under this section includes the furnishing of all labor, material and equipment required to mill existing pavement and provide new paving as specified hereinafter. 1.2 GENERAL REQUIREMENTS 1.2.1 Except as otherwise provided herein, materials and methods of operations required to install new and replacement pavements shall be in accordance with the applicable requirements and Standard Specifications for Road and Bridge Construction of the Florida Department of Transportation, 1999 & 1991 Editions. ~'' 1.2.2 Where construction requires removing pavement, or where existing paving is damaged by the Contractor's operation, it is the intent of these specifications that due care be exercised in cutting pavement, backfilling trenches and replacing pavement to the end of securing a condition where no further settlement of trenches will occur and the paved surfaces will be restored to a condition equal to or better than that existing before construction began. 1.2.3 No paving work shall be accomplished until all heavy construction equipment is permanently removed from the site. 1.2.4 All surfaces which have been injured by the Contractor's operations shall be restored to a condition at least equal to that in which they were found immediately before work was begun. PART 2 - MATERIALS 2.1 illin -Milling of existing asphalt pavement shall be performed to a depth of two (2) inches. All materials, equipment and workmanship shall be in accordance with Section 327 of the Florida Department of Transportation Specifications except that Paragraphs ~1 327-5 and 327=6 shall be omitted. , REQUEST FOR BIDS CONTRACT -MILLING AND PAVING FOR THE PUBLIC WORKS DEPARTMENT Page 2 of 4 2.2 Tack oat - A tack coat consisting of a bituminous material hereinafter specified shall be applied at a rate of between 0.02 and 0.08 gallon per square yard to the existing surface after milling and street sweeping. All materials, equipment and workmanship shall be in accordance with Section 300 of the Florida Department of Transportation Specifications except that Paragraphs 300-8 and 300-9 shall be omitted. 2.3 Asphaltic Concrete Surface Course. Type S-3 -The paving shall have a wearing surface of asphaltic concrete; 2" thickness to be placed in one lift on all milled roadways and 1'/z" thickness on all limerocked roadways. All equipment, materials, workmanship and methods employed on construction of the wearing surface shall be in accordance with Section 320, 300, 331 and 332 of the aforementioned Florida Department of Transportation Specifications, except that Paragraphs 331-6 and 332-7 shall be omitted. PART 3 -EXECUTION 3.1 Order ofWork -Work shall be accomplished in the following order: 3.1.1 Milling of 2" of existing asphalt pavement. 3.1.2 Tack Coat. 3.1.3 Asphaltic concrete surface course, 2" thick on all milled roadways and 1%" thick on all limerocked roadways. 3.2 Tests -Where reference is made in the Florida Department of Transportation Specifications for design mixes, tests of materials, or work performed, or where in the opinion of the Engineer, tests are required to ascertain compliance with the specifications, the Owner shall make such tests by an independent testing laboratory under separate contract. All testing expenses shall be borne by the Contractor. 3.3 Measurement and Payment - No additional payment shall be made for the work hereinbefore specified. Spoke to John Myers of Myers Contracting at (904) 261-3902 on April 15, 1999 at approximately 9:26 a.m. He stated he has used them for over a year and a half; they did new roads, parking lots and resurfacing for him. They are a great company and he has several more projects lined up for them to do. What few problems he has had were taken care of quickly, with no problems. 2. Left message for Dennis Close, Nassau County Road Supt. at -800-264-2065 at approximately 9:45 a.m. on April I5, 1999 to call me. He called back at 11:50 a.m. and said he was very satisfied with Milestone and that they had done over 200 tons of asphalt work (year-to-date) for him. 3. Spoke to Rex Lester, Street Supt. with the City of Fernandina Beach at (904) 277-7380 on April 15, 1999 at approximately 10:00 a.m. He stated he was very satisfied with Milestone and they have done asphalt and milling work for him.
