Chairman Don Wolfson called the meeting to order and asked for approval of the
minutes from the meeting held on August 15, 2000. On motion made and seconded the
minutes were approve.
1. Application for Use-by-Exception filed by Harvey K. Warnock to operate a
contractor's office at his residence located at 1812 Sea Oats Drive.
Mr. Warnock introduced himself and explained that he is a state certified general
contractor and desired to operate the office functions of the business from his home. He
stated that he had an additional business office in Jacksonville.
After discussion, Ms. Pillmore moved to recommend approval of the use-by-
exception with the strict application of the conditions listed in Section 24-159(a) and that
it be granted to the applicant only and for this location only. Mr. Burkhart seconded the
motion and it passed unanimously.
11. Application for Variance filed by William L. Paulk to allow construction of a
3-story residence to an existing nonconforming structure located at 336
Second Street.
Mr. Paulk introduced himself and stated that the owner, Leslie S. Frye, is his uncle
and they desired to construct an addition onto the existing residence creating a second
dwelling unit for his uncle. The existing structure is nonconforming in its side yard as it
does not meet the combined 15-foot requirement. He stated that the new addition is to be
three stories with a separate entrance through an existing breeze way. He added that the
addition will meet all required setbacks and height requirements.
After discussion, Mr. Burkhart moved to grant the variance and Mr. Jacobson
seconded the motion.
After further discussion and addressing issues regarding parking spaces and
construction specifications, the board voted unanimously to grant the variance.
Staff updated board on joint workshop issues and the actions taken.
There being no further business to come before the board the meeting was
adjourned.

Dear Mr. Frye:
This is a letter to explain our position on a contract made with Advanced Design
Systems to place an order for a metal roofing system to be installed at 336 2 d St.,
Atlantic Beach, Fl. 32233.
Once the 72 hour period after signing the contract had passed, the metal was
ordered obligating Advanced Design Systems to the metal provider for payment.
The metal was then shipped to Advanced Design System incurring shipping costs.
The metal was then shaped and formed at our production facility and a complete
metal roofing package was then shipped to 336 2nd St. where the shipment was
refused and sent back to our facility. At this time we were notified by Bill Pulk,
identifying himself as your Mr. Frye's nephew, that we would need to sit down with
him and provide him with additional information before we would be allowed to
begin work. A company representative, the Jacksonville office manager, promptly
set up an appointment with Mr. Pulk.
At this meeting the office manager, Dean Yancey, discussed with Mr. Pulk his
concerns and Dean assured Mr. Pulk that we could provide him with the
information he requested, that it would take a few days and would be back in touch
in a timely fashion. Mr. Pulk was convinced that we had satisfied his concerns up to
that point and assured Dean that he was not looking to break the contract.
In a timely fashion Dean called and left a voice message for Mr. Pulk letting him
know that he had acquired all the information he had requested. Mr. Pulk returned
the call and explained that we could meet in a few days. After a short time Mr. Pulk
called again to explain that Mr. Frye was moving into an assisted living facility and
in Mr. Pulk's terms, he needs to break the contract at all costs.
Dean contacted the home office in Orlando where he received all of the costs
associated with the project up to this point. The purchasing of the metal, the
shipment to Orlando, the man hours to fabricate the panels, then to fabricate the
trim, the loading of the materials for a complete roof system (including, but not

BACKFLOW PREVENTER INSTALLATION REQUIREMENTS
INSTALLATION: General installation instructions.
I. All devices larger than 2 inch must be installed on hard metal pipe. The metal piping
must include all components up to and including the first upstream and down stream
underground elbow joints.
2. Assemblies 2 inches and smaller may be installed on PVC pipe, Schedule 80 pipe
must 'be used near trafficked areas. Support brackets and guard posts are
recommended.
3. Device assemblies are to be installed a minimum of 12 inches above flood grade.

Measurement is to be made from the lowest point of the device. The maximum heigh
of a horizontally installed assembly is not to exceed 60 inches. No vaulted
installations are allowed.
4. Vertical installation of DCAs is permitted on fire risers only and only if approved by
USCFCCCHR.
5. All Devices are to be installed in a manner to facilitate testing. All valves must be
fully operational. All test cocks must be easily accessible.
6. Freeze protection is required for Fire Suppression System Backflow Preventers and is
recommended for all other Backflow Preventer installations. Such protection may not
prevent the unit from operating or being tested. All Backflow Preventers must be
inspected by a COJB representative prior to application of freeze protection.
7. Thermal expansion must be addressed in situations were a device application may
restrict such expansion.
8. AVBs and PVBs must be installed a minimum of 12 inches above the highest outlet
served.No valve may exist downstream from these devices.
