This letter is provided in response to the City's Request for Additional Information(RAI)dated December
16, 2014. Plans documenting these changes are included for your review and approval.
To assist with review of this submittal, the comments have been restated in bold followed by Genesis'
response.
DK, CW,MC, TNT Comments:
1. If the plan modification that was approved (stop directional drill at SW property line and
construct open-cut WM along pond w/ easement) will be done, need modified plans, and
easement docs.
Proposed off-site water main has been revised as discussed. The amended plans are included for
your review.
2. Profile drawings show water main going under storm at various locations at depths of 5—8
feet, but in no Case A or Bs noted on C-07. Is water main proposed to be at those depth for
the length? Wherever possible, have water main over storm, and where it goes under storm
it should come back up to reasonable depth-3 feet.
The design intent is for the potable water to cross over the storm pipe wherever feasible. In order
to maintain 36"of cover as well as 12"of vertical clearance from storm pipes, the water main must
cross beneath the storm pipe when the storm pipe cover is below 5'. In these instances, the pipe
is proposed to be deflected vertically beyond the crossings so that the amount of deep burial is
minimized.
3. Repeat—Use detail for hinged manhole covers.
The hinged manhole cover detail has been added to sheet C-22 pursuant to your request.
TAMPA TALLAHASSEE PENSACOLA OCALA JACKSONVILLE CRESTVIEW
Rick Carper Comments:
1. Repeat—Lot 11 and Lot 17 Easements(?) between ST-2 and ST-1, and ST-13 and ST-6—
width as shown is inadequate. COJ LDR 4.4.6 requires minimum 20' width, plus 2' for
each additional foot of cut below 4'. Estimate width should be 28' between lot lines for
Lot 11 and 22' for Lot 17. Alternative is to plat unobstructed utility easement on adjacent
lots.
It is understood that Section 24-254 of the COAB code requires easements to be a minimum of
15' wide and extend from street to street. Regardless, additional 5' wide drainage easements
have been added to the lots either side of the referenced public tracts (lots 11 and 17). This
should address the City's concern regarding this matter.
2. Repeat— FDOT storm manhole approx. 150' from SW property corner appears to have
no purpose when pipe from property plugged. Coordinate with FDOT to see if manhole
can be capped / abandoned when new sidewalk poured over this location (Not addressed
in FDOT Connection Permit Application).
Connections from site have already been coordinated with FDOT. Burying manhole beneath
sidewalk would make it difficult to locate should maintenance or extension of this line be needed
in the future.
3. Plan uses four FDOT Type 9 curb inlets. From storm tabulation, runoff to Type 9's
exceeds inlet capacity as shown in FDOT Index 214. May require use of Type 4 or COJ
City Standard(preferred) inlets.
Referenced inlets have been changed to COJ City Standard pursuant to your request.
4. ST-18 Closed Flume Inlet being constructed in segment of FDOT Type A (16") curb.
Index 216 detail referenced uses FDOT Type F (24") C&G. Need detail showing curb
modifications. Note: 216 calls for 6'6"sidewalk above if located at back of curb as in this
installation.
The construction plans have been revised to include a 3'transition from Type A to Type F curb
on either side of the Closed Flume Inlet. Additional notes have been added to sheet C-09
instructing contractor to match back of curb in transition section. The Closed Flume inlet is
currently shown with a 6'6"sidewalk in accordance with Index 216.
5. Bars in MES grates, if constructed per Index 273, are 14" center to center. May not
provide the protection desired in this usage. Grate on Flared End Section not addressed
in FDOT Index, detail required.
So noted, the MES grates on ST-17 have been modified to reduce spacing to 10"O.C. to block
the passage of balls within the park. Grate reference on ST-11 was erroneous and has been
removed.

indicated so that the applicant can submit this application package to FDOT for approval.
In addition, four copies of each the FDEP permit applications for water and wastewater system

One thing that we typically do in other municipalities is check the water flow near or connection point. As we are
