In the event it is necessary for the CITY or the City's approved representative or other franchised utility to enter
upon the above described easement or property of the CITY, the USER shall replace at the USER's sole expense,
any and all material necessarily displaced during the action of maintaining, repairing, operating, replacing or
adding to of the utilities and facilities of the CITY or franchise utility provider.
The facilities allowed by the permit shall meet the current requirements of the City Code, Building Codes, Land
Development Code and all other land use and code requirements of the CITY, including City Code Section 19-7(h)
which states"Driveways that cross sidewalks: City sidewalks may not be replaced with other materials, but must
be replaced with smooth concrete left natural in color so that it matches the existing and adjoining sidewalks."
The USER, prior to making any changes from the approved plans and/or method, must obtain written approval
from the City of Atlantic Beach Public Works Department, for said change within 30 days after the day of
completion.
This permit shall inure to the benefit of, and be binding upon, the USER and their respective successors and
assigns.
USER shall meet the terms and conditions of this permit and to all of the applicable State and CITY laws and/or
specifications, to include utilities locate requirements and use limitations/requirements of easements, public
right-of-ways and other public land. USER further agrees that the CITY and its officers and employees shall be
saved harmless by the USER from any of the work herein under the terms of this permit and that all of said
liabilities are hereb assumed by the USER.

Remove and replace concrete or grass with pavers.
PAYLESS BRICK PAVERS, LLC, will supply all associated materials and install approximately xxx sq ft
of pavers. Should customer request and the contractor agree that additional work is performed beyond the
agreement; the final invoice will reflect the cost of any additional work. A final field measure upon
completion will be performed and the homeowner will only pay for the material used and installed. The
price reflected in this proposal includes basic pavers, for special orders the price may vary. Basic pavers:
Old towne, Mega old towne, Stonehurst and 4x8 pavers any color.
As of Aug 12th-November 30th we will only offer the following colors Sierra, Sand Dune, Glacier,
Autumn, Granite and Lions Bridge in Mega Old Towne, Reg Old Towne, 2 PC, Stone Hurst and 4x8's
MATERIAL:
The pavers to be installed are follows;

Based on 800 sq ft of pavers @ $ 4800 driveway
Based on 125 sq ft of pavers @ $ 1000 pavers stripe on center of driveway

Installation Procedures:
Border pavers are mortared: field pavers are set on the compacted base and the joint will be filled with sand.
Polymeric sand available extra charge
A small amount of sand will be left on top which homeowner will clean sweep within a week of installation.Do not step
or walk on borders for 48 hours after installation.
Terms:
Homeowner to supply running water and electricity.
Leftover materials beyond contracted square footage (pavers, coping) are property of
contractor unless homeowner wants to purchase additional pavers for his use, the homeowner will only pay for
the cost of installed pavers .
Homeowner will retain a few pieces for future replacement at no cost
Payless Brick Pavers is not responsible for damages to underground utilities, sprinkler system
unless the homeowner lets us know the location),landscaping light, damage on plant or grass. Patio and pool furniture
will be removed by the homeowner before installation),color variations in pavers, and damages by other contractors.
Permits and fees, if needed; are the responsibility of the Homeowner ( City and HOA); Payless
Brick Pavers will sign all applications for the homeowner as needed.
Cleaning dust from windows,furniture,screens,etc,after installation is the responsibility of the
homeowner. Dust generated by the installation is unavoidable.
If the homeowner does not pay full contracted amount after 7 days of installation date the account will be considered
past due and homeowner shall pay interest of 1.5%on the remainder of balance.
Payless Brick Pavers requires a 50% deposit when the contract is signed.
Balance to be pay 7days after completion installation by check, cash or credit card*. If payment is not received
between 7 days after installation date a five-day grace period is given to the homeowner, after that a notice to Owner
or Contractor will be mailed to the homeowner or contractor and if the payment is delayed beyond that, any and all
collection cost will be incurred by the homeowner, including attorney and court fees.
Prices quoted are based on cash or check;the above pricing and terms of sale are acceptable and are hereby agreed to by
homeowner or contractors. Payless Brick Pavers warranties labor only for 2 years from installation date.Warranty
does not cover damage done by any natural disaster such as a flood,hurricane or fire.
This proposal expires 30 days from 10/21/2020.
